What are the responsibilities and job description for the Home Health Nurse Practitioner position at Optum?
Optum is looking for a Full-Time Home Health Nurse Practitioner to join our team in the San Gabriel Valley. The clinicians we seek are those who practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Position Details
- Provide long term primary care for home bound patients not be able to access medical care outside their residence
- Provide short term care management (~30 Days) for patients recently discharged from the hospital in collaboration with their Primary Care Provider and Specialists
- Skillset for providing care to geriatric/adult patients is essential
- 8am-5pm Mon-Fri
- 6-8 patient encounters/day
- No nights, weekends
- Call required - starts at 5 pm on work days and all day on weekends and holidays
Requirements
- Master’s degree from a four-year college and/or a professional certification beyond a four-year college
- Certificate of completion from an accredited Nurse Practitioner or Physician Assistant Program
- Unrestricted CA Nurse Practitioner or Physician Assistant license
- Current CA DEA certificate required prior to start date
- Previous experience in either ambulatory care, home health, SNF or a similar environment
- Comfortable seeing adult/geriatric patients and willing to render care in a home environment setting
- EMR Proficient
- Full COVID-19 vaccination is an essential requirement of this role. UnitedHealth Group will adhere to all federal, state, and local regulations as well as all client requirements and will obtain necessary proof of vaccination prior to employment to ensure compliance.
Preferences
- Critical Care, Palliative, or Geriatrics certification as a Nurse Practitioner
- Previous experience in a managed care setting and working with medically complex patients
- Medical Spanish preferred
