SHIFT/PASS DAYS: 7:30am – 4:00 pm, Monday-Friday

 

OTHER: *Posted Hiring Salary includes the sum of statutory hiring rate ($49,202) and Mid-Hudson Adjustment ($1,513). The Mid-Hudson Adjustment ($1,513) is also added to job rate ($62,806).

 

POSITION DESCRIPTION:

The HS1 at the Amenia Program Center Orchard Classroom is a necessity to ensure continuation of day habilitation services for 7 individuals from the Orchard ICF and is essential for providing opportunities for socialization to populations with high bs essential for providing opportunities for socialization and job training to special populations and high behavior needs individuals who cannot be served by voluntary agency programs.

 

PRIMARY DUTIES AND RESPONSIBILITIES:

Duties will include, but are not limited to:

• Writing and updating Objective and Service plans for individuals at least annually to reflect current CFA goals and needed services.

• Completing assessments at least annually, and providing quarterly review updates, as well as completing monthly progress notes.

• Maintaining records on said plans, as well as updating progress of the individuals’ development in skill areas. This includes ensuring service delivery compliance, data collection forms, monthly summary notes, and billing documentation are all completed in a timely manner and within standards.

• Ensures respectful relationships among staff, among persons served, and among staff and service recipients in a manner consistent with OPWDD’s guiding principles of putting the person first and improving overall quality of life so that the service recipients’ goals and objectives may be realized.

• Instructs, guides, and supervises staff in the program area who are responsible for employing proper strategies with the individuals that are consistent with their individual plans of care, while providing clinical guidance.

• Works with the treatment team in maintaining and assisting current treatment plan as well as with a future community placement plan.

• Attends all treatment related and team level meetings including but not limited to Annual Case Review meetings, Quarterly meetings, Special ITT's, placement meetings, etc.

• Maintains trainings to staff on all treatment plans for which Habilitation Specialist is responsible for

• Provides effective instruction/training to direct care staff in implementing service plans and other relevant issues to enable staff to deliver more skillful assistance to the service recipient.

• Ensures direct care staff appropriately perform habilitation service tasks.

• Directly implements some individuals’ goals and completes appropriate documentation.

• Provides oversight of direct care staff to ensure they implement goals and services and document appropriately.

 

NOTE: You must have a valid driver’s license to apply as travel is a part of the duties involved in this position. This may include a state-owned vehicle, as available, or the employee’s own car. An employee who uses his/her own car will be reimbursed at current negotiated rates.

 

MINIMUM QUALIFICATIONS:

Permanent Qualifications: Must have QIDP* certification/status and permanent service as a Habilitation Specialist 1

OR Must be active and reachable on the current Civil Service eligible list for the Habilitation Specialist 1

 

Transfer Qualifications: Be eligible for a Section 70.1 transfer into a Habilitation Specialist 1 title

 

Provisional Qualifications:

Departmental Examination: Candidate must be a qualified employee of the New York State Office for People with Developmental Disabilities and have had one year of permanent competitive or 55-b/55-c service in a clinical or direct care position AND must also possess a bachelor’s degree or higher in a human services field OR permanent New York State certification as a teacher.

OR

Open-Competitive Examination: Candidate must have a bachelor’s or higher-level degree in a human services field AND one year of direct care or clinical experience where your primary focus is providing services to individuals with developmental disabilities.

Substitution: A master’s degree in a human service field OR permanent New York State certification as a teacher may be substituted for the degree and experience described above.

 

Preferred Qualifications:

* QIDP (Qualified Intellectual Disorder Professional) designation requires one year's experience working with people with a developmental disability AND a Bachelor's Degree or higher in a human services field (including, but not limited to: sociology, special education, rehabilitation counseling or psychology).
