The Wachusett Respiratory Care Unit at Holden Rehabilitation and Skilled Nursing Center has openings for a part time Certified Nursing Assistant. The Respiratory Unit is a specialty unit providing care to patients with ALS, Traumatic Brain Injury and Ventilator Dependency. The hours are 7A to 7P, 24 hours per week Week 1 is Tuesday and Saturday; Week 2 is Sunday and Thursday. The desired candidate will have a passion for caring for residents needing 24 hour critical care in a long term care environment. Position Summary:
Provides direct care to nursing home residents, primarily for their activities of daily living, but also to include some medical care.
Qualifications
Educational: State certification required within 120 days of hiring. CEUs (Continuing Education Units) as required to maintain certification.

Ability to perform the essential functions:
In this position, the employee is required to be on his or her feet continually throughout the day. On a full time, nonrestrictive basis, the employee must be able
to lift frequently over 50 lbs. of weight, which is required when transferring a resident from a bed to a wheelchair. The use of a transfer belt is necessary and
required when transferring any resident. This is for the safety of not only the resident, but also the Nursing Assistant. Other Nursing Assistants or devices are at
times available to assist with heavier residents if needed. Must have the ability to perform essential functions without posing a “direct threat” to patients or the staff.
Essential Functions:
1. Assists residents routinely and as needed with any and all activities of daily living, including oral hygiene, bathing, grooming, dressing, toileting, nutrition and feeding,
exercise and mobility, transfer and ambulation, transportation and activity.
2. Receives instructions and guidance from the licensed nurse(s) on the unit regarding individual resident needs.
3. Consults resident’s written plan of care to obtain information and instructions regarding individual resident’s needs.
4. Performs assigned tasks according to established procedures and practices including:
a. Gathering and recording data, e.g. – vital signs, weight, intake and output, specimen collection, etc.
b. Carrying out nursing routines. e.g., to prevent decubiti, contractures and immobility and to keep patients clean, comfortable, safe and secure, etc.
c. Providing restorative nursing care, e.g., range of motion, positioning, preventative skincare, reinforcing teaching and providing assistance with activities of daily living, exercise and activity, etc.
d. Assisting in maintaining the safety and cleanliness of the resident’s total environment.
5. Makes observations of resident’s physical and mental state and reports observations and changes to nurse in charge including mental status, behavior/mood, vital signs, weight, skin condition, mobility status, appetite/nutrition, activity level, level of independence in activities of daily living, and progress in restorative nursing programs.
6. Reports and records accurately all current resident observations and information in resident’s records and on other forms as provided.
7. Communicates with appropriate staff regarding all aspects of resident care and seeks assistance as necessary.
8. Respects resident rights (i.e. knocks on door before entering room, addresses resident by name, allows resident to make decisions).
9. Attends and participates in in-service and continuing education programs for growth and development.
