Expected pay for this position is $29.61 - $34.84/hour. Actual pay will be determined by experience, skills and internal equity.
POSITION SUMMARY:
The Trauma Recovery Case Manager is responsible for identifying, assessing, planning, coordinating, implementing, monitoring and evaluating options and services across the continuum of care. Responsible for coordinating on behalf of the patient the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services. The case manager actively communicates with the team to facilitate and expedite the best outcomes for the patient. Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. The Case Manager works with the patient and treatment team to help establish goals and realize optimal outcomes as determined by current individualized care plans.
Qualifications
REQUIRED QUALIFICATIONS:
Education:
Master's Degree in social work or other related field
Experience:
One year of experience care management, patient care, or related
Other Licenses/Certifications:
American Heart BLS or obtain within 90 days of hire
Other skills or knowledge:
Interpersonal skills in order to engage, coach, and mentor others, facilitate/organize/direct the work of others and to maintain positive relationships among participants, leadership, ministry/facility staff and internal/external stakeholders
Proven skills in written/verbal communications, problem solving, independent problem solving, decision making abilities, creativity, prioritization/delegation and necessary computer-related skill
PREFERRED QUALIFICATIONS:
Licensure/Certifications:
Current state of Illinois licensure as a LSW (Licensed Social Worker) or LPC (Licensed Professional Councilor)
Certified Clinical Trauma Specialist Certification will be required and provided by OSF within first 90 days of employment
Other skills/knowledge:
Working knowledge of trauma, including effects of trauma, types of trauma, key elements of working with trauma and evidence-based treatment approaches
