Job Overview: We are seeking compassionate and dedicated Home Health Aides to join our hospice care team. As a Home Health Aide, you will work closely with patients and their families to provide personal care and companionship. Responsibilities:
- Provide personalized care to hospice patients, including assistance with bathing, grooming, dressing, and toileting.
- Assist with mobility, transfers, and positioning to ensure the comfort and safety of the patient.
- Monitor and report changes in the patient's condition to the nursing team.
- Provide companionship to patients and their families.
- Maintain patient confidentiality and privacy at all times.
- Document care provided and any changes in the patient's condition accurately and timely.
- Follow all company policies and procedures regarding infection control, safety, and HIPAA regulations.
- Adhere to the customized care plan.
Qualifications:
- High school diploma or equivalent.
- Completion of a state-approved Home Health Aide training program.
- Valid Home Health Aide certification.
- Minimum of 1 years of experience in hospice or home health care.
- Knowledge of infection control, safety, and HIPAA regulations.
- Compassionate, patient, and respectful attitude toward patients and their families.
- Ability to work independently and as part of a team.
- Reliable transportation and valid driver's license.
Working Conditions: This is a full-time or part-time position that requires flexible availability. Home Health Aides may travel to multiple patient homes per day and must be comfortable working in a variety of home environments.
