Job Overview: We are seeking compassionate and dedicated Home Health Aides to join our hospice care team. As a Home Health Aide, you will work closely with patients and their families to provide personal care and companionship. Our hospice company has a focus on privacy, dignity, and customized care for each patient and their family, and we strive to provide the highest level of personalized care in the comfort of their own homes.

Responsibilities:

  • Provide personalized care to hospice patients, including assistance with bathing, grooming, dressing, and toileting.
  • Assist with mobility, transfers, and positioning to ensure the comfort and safety of the patient.
  • Monitor and report changes in the patient's condition to the nursing team.
  • Provide companionship to patients and their families.
  • Maintain patient confidentiality and privacy at all times.
  • Document care provided and any changes in the patient's condition accurately and timely.
  • Follow all company policies and procedures regarding infection control, safety, and HIPAA regulations.
  • Adhere to the customized care plan.

Qualifications:

  • High school diploma or equivalent.
  • Completion of a state-approved Home Health Aide training program.
  • Valid Home Health Aide certification.
  • Minimum of 1 years of experience in hospice or home health care.
  • Knowledge of infection control, safety, and HIPAA regulations.
  • Compassionate, patient, and respectful attitude toward patients and their families.
  • Ability to work independently and as part of a team.
  • Reliable transportation and valid driver's license.

Working Conditions: This is a full-time or part-time position that requires flexible availability. Home Health Aides may travel to multiple patient homes per day and must be comfortable working in a variety of home environments. Our hospice company has a focus on privacy, dignity, and customized care for each patient and their family, so Home Health Aides must be able to adapt to each patient's unique needs and preferences and provide a high level of personalized care.
