Overview
While working as a Hospice Registered Nurse, you will provide care to Hospice patients and support to their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws and agency policies and procedures. May provide the initial assessment and comprehensive nursing assessment for admissions, as well as ongoing visits as needed. Provides input to the Interdisciplinary Team (IDT) plan of care. Works closely with team ensure all patient and family care needs are met by reporting at the close of shift. Communicates the patient’s progress with the patient/caregiver and other disciplines.

Qualifications
- Education: Bachelor’s degree in Nursing preferred. Graduate of approved nursing program
- Licensure/Certification: Certified Hospice and Palliative Nurse (CHPN) certification preferred.
- Experience: A minimum of 2 – 4 years’ direct patient care experience in hospice, LTC or home care
