Essential Functions
- Promotes ongoing participation of the patient with decisions involving plans, goals and progress.
- Completes adult comprehensive assessment, admission MIS form, and treatment plan within first 24-48 hours after admission.
- Provides a minimum of one (1) psycho-educational group per day.
- Coordinates discharge date with nurse and completes discharge summary and discharge MIS forms.
- Maintains relevant and timely counseling progress notes.
- Attends daily report with other case managers and communicate patients’ plans and progress as coordinated with team members.
- Assists in team effort in handling crisis-oriented emergencies. Documents all interventions in the counseling progress notes.
- Attends staff meetings, supervision, in-service conferences, periodic reviews and other relative meetings as determined by the clinical director.
- When possible and/or requested, provides referrals for affected family members to self-help programs.
- Collaborates with other professionals to evaluate needs of patient.
- Maintains documentation of records which meet Dimock, regulatory, and Joint Commission standards.
- Assesses patient’s continuing care needs, contacts appropriate aftercare resources, and arranges for transfer or admission to appropriate rehabilitation facilities and/or liaison with self-help organizations.
- Formulates a treatment plan based on comprehensive bio/psycho/social assessment of patient. Reviews and revises treatment plan based on clinical needs and regulatory bodies. Completes required documentation within 72 hours.
- Completes case closings. Conducts post-discharge follow-up for clients within 30 days of discharge.
- Completes all encounters for patients seen the day of before ending business day.
- Completes all notes for individual and group sessions. Files patient charts at the end of each business day in the nurse’s room.
- Utilizes clinical skills to create a positive working relationship with patients in order to facilitate desired change using appropriate clinical models.
- Develops supervision plan with Clinical Supervisor and meet on a regular basis to discuss clinical, organizational, and agency issues and review performance.
- Demonstrates working knowledge of Dimock’s continuum of care and facilitates appropriate referrals/contacts within the larger system.
- Demonstrates knowledge of and adherence to initiatives in Corporate Compliance and HIPAA.
- Other duties as assigned.

Work Environment
Acute Treatment Services (ATS) is a 35-bed medically monitored detox unit for alcohol and opioid use disorders. This position
works in a clinical/office environment, on the Dimock Campus.
Physical Demands
This position requires continuous standing, sitting, and walking for long periods of time. Writing, typing, and looking at a
computer screen are frequent actions.
Position Type/Expected Hours of Work
Varies. Must be available for weekend and holiday coverage.
Required Education and Experience
- Associate's Degree in Social Work or Counseling, and completed certification/degree program in addiction counseling or equivalent required.
- At least one year working in a substance use therapy program required.
- Bachelor's or Master’s in Counseling (LICSW or LMHC) or a License Addiction Drug Counselor (LADC1) preferred.
- Bilingual in English/Spanish or English/Creole preferred.
