Job Description
As a Caregiver for Outreach Health, you are responsible for helping clients that are disabled or elderly by assisting with daily tasks, basic health related tasks, and provide companionship. To ensure the client is properly cared for, you will understand and perform basic safety techniques.

Essential Job Functions
Supports and promotes self-care and independence of the client(s); duties may include but are not limited to: bathing, dressing, grooming, feeding, toileting, cleaning, laundry, meal preparation, transfer/ambulation, escort, routine hair/skin care, exercise, shopping and assistance with self-administered medication, respite, protective supervision and/or habilitation services (with additional training).
Must complete tasks and/or activities during scheduled, authorized hours and reports by Electronic Visit Verification/Fixed Visit Verification (EVV/FVV) system when arriving and leaving the clients home each day worked (as directed by the Field Supervisor or CMPAS Assessor of Need)
Reports any significant changes in the clients circumstances to the Field Supervisor immediately, e.g. vacation, hospitalization

Required Skills, Education, Certification, License, and/or Experience
Must have a High School Diploma, GED, or equivalent work experience. If the applicant is under the age of 18, the applicant must be a High School graduate or enrolled in a Vocational Educational Program
Must pass a criminal background screening
Must be able to follow verbal and written instructions
Must be flexible to service several different clients a day
Must be able to work with limited supervision and work on weekends and holidays
Must comply with HIPPA, applicable healthcare laws and statues (local, state, and/or federal)
Maintain a valid drivers license and own a legally registered vehicle that holds the minimum liability insurance coverage. (Required for caregivers that drive only)
In the Primary Home Care or Community Attendant Services Program, the caregiver cannot be the spouse of the client; Also, the caregiver cannot be a foster parent or a parent of a minor that receives state or federal funded services. However, in the CMPAS program, the Caregivers may be the parent, sibling, or spouse of the client.
Preferred Skills, Education, Certification, License, and/or Experience
Bilingual (English and Spanish)
Prior experience as a caregiver or similar role
CPR and First Aid certification (required for CLASS, MDCP, DBMD)
Working Conditions and Physical Requirements
Must be able to work in a work environment that involves intermittent body movements such as walking, standing, sitting, driving, bending, lifting up to 50 pounds, stooping, kneeling, climbing stairs, and reaching. Must have dexterity and strength in hands/limbs for reasonable accommodation to perform required tasks. The Nature of duties presents moderate exposure to infectious and chemical hazards or job-related accidental injury. Must be able to travel to perform field visits or travel between offices if necessary. Not all sites/locations will be handicapped accessible. (Driving may be a requirement depending on duties and responsibilities).
