Outside In is looking for a dynamic and adaptable project-focused team player to join the Clinic Services team as our Population Health Outreach Coordinator. This position is an important member of the Clinic Services outreach and community wellness work. This role will coordinate workflows and systems to outreach to patients with the goal of improving patient engagement with their Patient-Centered Primary Care Home. The work includes preventative outreach, re-engagement, and data analysis.  The Population Health Coordinator will work at our Downtown offices located at 1132 SW 13th Ave, and reports to the Quality Improvement Manager.

This is a full-time, 40-hour-per-week, union-represented, non-exempt position with a starting hourly pay range of $20.16- $21.97, depending on experience and skills. What you will do:   

  • Develop sustainable workflows and processes to communicate with patients and enable them to engage in their primary care
  • Develop communication channels, platforms, and systems to improve outreach to patients.
  • Coordinate outreach activities between MAs, volunteers, RNs, Panel Managers, and Outreach Workers, as well as other teams to support and engage patients and underserved folks facing barriers to their healthcare.
  • Conduct outreach activities to link individuals with their primary care provider and improve health literacy related to the importance of preventive health visits and the benefits of primary care services.
  • Collect and analyze data on the effectiveness of contacts, interventions, and outcomes.
  • Utilize motivational interviewing and coaching techniques to remove barriers to care and engage patients in their own individual care.

EDUCATION AND EXPERIENCE

Required:

  • Proof of COVID-19 vaccination status or approved medical or religious exception on file is required
  • Two years experience working in a community social service or healthcare environment.
  • Demonstrated ability to adhere to trauma-informed care principles and apply motivational interviewing techniques.

Desired but not required:   

  • Bilingual English/Spanish preferred.
  • 4 years or more experience or a Bachelor’s degree or higher in health or another social service field of study. 

WORKING CONDITIONS  

This position is in a typical office and clinic environment, with moderate noise. Potential for a hybrid work schedule with some work from home.

This job requires operating phones, computers, and other office equipment. Communicating is required on a regular basis. Moving inside the building to other offices and program delivery spaces as well as moving to surrounding sites is expected. Sporadically moves boxes or items weighing up to 10 pounds.
