Oyster Bay Assisted Living facility is seeking a compassionate and dedicated Case Manager. We have our eye out for a candidate with a proven track record of successfully coordinating services for residents, while remaining committed to excellence in resident satisfaction. The chosen candidate will have the opportunity to work with positive and supportive team of healthcare professionals.
Job Responsibilities:
- Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
- Organizing comprehensive service plans and assessments with the residents and their families
- Actively participate in the admissions process and will work closely with family members to ensure the needs of the resident are met
- Assist the resident in transitioning to their new home and routine.
- Partner with interdisciplinary care teams to ensure the resident's physical and social needs are met
Qualifications/Requirements:
- Prior experience in Long Term Care a PLUS or 1 years’ experience as a Case Manager/Social Worker
- Strong organizational skills, passion and compassion for the population
- Ability to work cooperatively with multidisciplinary team
- Bachelor's Degree is preferred
- professional, dedicated, detail oriented and empathetic to the special needs of the elderly

Pay: $65,000.00 - $78,000.00 per year
