The Assertive Community Treatment (ACT) Registered Nurse (RN) provides nursing services to adult patients enrolled in the ACT program that are interested in improving their health and wellness. RN assesses physical and psychiatric health strengths, needs, and interests as it applies to psychiatric nurse management, makes appropriate referrals to community physicians, provides management and administration of medication in conjunction with the ACT psychiatric provider, provides information and education, adds health promotion goals/objectives/interventions to treatment plans, and accurately documents progress via progress notes. RN provides cross-training and consultation to the treatment team about nursing services and health promotion as interventions for any patient interested in health promotion and intervention.
