Case Manager - Permanent Supportive Housing
Position Title: Permanent Supportive Housing Case Manager
Hours: Full-time, Monday through Friday 8:00 am – 5:00 pm
Organization Summary: Problems Anonymous Action Group (P.A.A.G.) is a non-profit agency which provides housing, socialization, case management, and work approximation to Seriously Mentally Ill (SMI) and chronically homeless individuals in Ogden Utah. It is our mission to advance human dignity for people on the fringe of society through meaningful involvement.
Position Summary: This position is responsible for managing an assigned caseload of clients having long-term mental illness. Assessing client needs, developing, implementing and reviewing service plans. Also working with other community resources in meeting and achieving client services and housing needs.
Essential Duties and Responsibilities:
- Build positive, appropriate, and ethical relationships with each client (member) on caseload.
- Provide intake and quarterly assessments with individuals to appropriate community services, assist in securing permanent housing.
- Assist clientele in creating an Individual Service Plan (ISP) to ensure personal goals and necessary life skills are being developed to help improve self-reliance.
- Plan and hold weekly socialization activities with members in our Drop-in Center.
- Complete in-home inspections with clientele to assist with housing compliance and other daily needs.
- Maintain accurate case records and provide aid with appropriate documentation through the online database.
- Work closely with community partners to facilitate care coordination and information sharing.
- Attend coordinated housing meetings to discuss and report sensitive and confidential information.
- Meet regularly with immediate supervisor as a means of enhancing professional growth, reviewing, and processing the provision of case management services, and dealing with appropriate administrative issues.
- Attend regularly scheduled team meetings and team training.
- Provide transportation for members.
- Food handlers permit and basic cooking knowledge to assist with daily meals and program events as assigned.
- Other duties as assigned.
Qualifications: To perform this job, an individual must be able to perform each essential duty and responsibility. Employment is contingent on successful completion of a criminal background check. Valid Utah driver’s license, automobile insurance, and safe driving record is required.
Education and/or Experience: Must possess a combination of mental health work experience or relevant education, and training which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties. Commitment to the highest standard of ethics and confidentiality. Two years of case management experience and/or a Bachelor’s degree in relevant human services is preferred.
Abilities: Demonstrated ability to interview and assess clients, using appropriate assessment tools, and observe, record and report on an individual’s functioning; to read and understand assessments, evaluations, observation, and use in developing treatment plan; to identify community resources and services for clients and coordinate provision of services; to establish effective working relationships with internal agency staff as well as with relevant community organizations; interact positively with consumers and work as a team member, communicate effectively, verbally and in writing, to maintain strict confidentiality, and to work independently under general supervision is required.
