POSITION: Regional Liaison
REPORTS TO: Program Manager
MAIN LOCATION: 2855 N. Keystone Ave., Indianapolis, IN 46218
HOURS: 8:30 – 5:00
POSITION DESCRIPTION:
The Regional Liaison is responsible for being the point person for referred clients by assisting them in locating and navigating recovery support services to ensure a successful connection to needed services. The Regional Liaison must be knowledgeable of community resources, health care plans and services to connect referred clients to an optimal continuum of care by utilizing PACE’s care- coordinated model for case management and referral services.
Liaison positions will work remotely but must be willing and able to meet physically at PACE’s main location for team meetings, etc.
POSITION REQUIREMENTS:
- Must have experience and/or a working knowledge of the criminal justice system, mental health care and recovery services
- Must have experience utilizing teleconferencing technology such as ZOOM, Microsoft
Teams, etc.
- Must have experience with data collection and reporting including the utilization of
Microsoft Excel.
- Strong attention to detail and exceptional customer service delivery
- Experience with working with diverse populations within a community, social service, or health care setting
- Excellent Communication skills
- Ability to work within a team environment as well as working well independently
- Strong data entry experience is required
- Regional Liaisons must not have any contracts and/or agreements with referring agencies to avoid conflict of interest
POSITION RESPONSIBILITIES/DUTIES:
- Interview clients seeking services in a compassionate manner, primarily utilizing telecommunication devices and data management utilization effectively.
- Refer clients to appropriate community resources to provide a continuum of care and supportive services within 24-48 hours.
- Ensure follow up calls are made to clients following all referrals made.
- Ensure accurate client tracking into data management systems following referrals.
- Encourage and motivate clients to follow their service recommendations.
- Provide monthly reporting to the Program Manager on number of referrals made including status of referrals, client demographics, etc.
- Provide monthly reporting to funding and referral providers for outcome measures achieved.
- Create tracking spreadsheet of referrals and successful connections to community service providers.
- Provide crisis intervention, information and referral resources, and case management services as needed.
- Assess needs of clients to determine appropriate referral service delivery.
- Answer all correspondence from clients regarding services needed.
- Remain up to date on community agencies in assigned region, including status of available services, eligibility requirements, etc.
- Ensure contact communication from criminal justice referring agency (i.e. probation/parole officers, etc.) regarding status of referrals.
- Provide referral case notes for every intervention (i.e., appointment, phone call, etc.) with clients into the client services database as required.
- Attend all meetings and trainings held in the community about the re-entry population.
PACE, INC. RESPONSIBILITIES:
- Always represent PACE in the most professional way especially when in the community.
- Attend and actively participate in all staff meetings.
- Dress, always speak and conduct yourself in an appropriate and professional manner.
TRAINING:
- Attend trainings when necessary.
- Remain educated about service agencies in assigned region.
OTHER:
- All other duties as assigned.
