Summary :
Responsible for the developing and organizing the plan of care for participants ensuring continuity of home care service delivery.
Other information :
Minimum of one year experience working with the frail and elderly. Prefer two years of experience working in home health agency, community-based or long-term care setting involving the elderly.
Excellent verbal and written communication skills. Ability to work without close supervision and to exercise independent judgment.
Ability to organize multiple tasks and projects and maintain control of work flow. Must be medically cleared of communicable diseases and up to date with immunizations.
All experience must be in the last 5yrs
This job will be authorized 80.00 hours bi-weekly.
