Salary Range: $120,000.00 per year with quarterly bonus opportunity

A Internal Medicine Group has an opportunity for a Full Time Advanced Practice Registerd Nurse (APRN) or Physician Assistant (PA) to work with patients at home in the Wallingford area. This is an excellent opportunity for those that enjoy a flexible work schedule,working autonomously, and accelerate in a pay for performance environment.
The APRN or PA will travel to patients homes to provide care.
Essential Function
- Visiting patients in the home to assess, care for and and create a care plan
- Leading the Home Health Clinical Team on patient care in the home
- Create a care plan for the patient
- Utilize EHR, Epic to update patient chart
- Engages patients and their families or caregivers in understanding, setting and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.
- Analyze patients medical history, medication allergies, physical condition and examination to determine best care
- Diagnose medical condition
- Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
- Directs and coordinates the patient care activities of nursing and support staff as required.
- Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards
Education and Experience
APRN
- The Advanced Practice Registered Nurse is masters or doctorate prepared
- Certified Nationally as a Nurse Practitioner through AANP or ANCC .
- The Nurse Practitioner must have possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a y) Nurse Practitioner by an approved certifying body.
- DEA, and Ct controlled substance license
- Experience working with elderly patient population
- Experience working with EHR
PA
- Physician Assistant Certification
