Position Details:
- Full-time Hospice LPN Position
- Schedule: Monday thru Friday, 8a-5p
- Available territory openings: North-West Indianapolis, Boone County

Job Functions:
- Performs physical and psychosocial assessment on all hospice patients.
- Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
- Assesses patient’s condition and coordinates with RN Case Manager.
- Participates in case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.
Requirements
Experience:
- One year Hospice experience preferred, but not necessary
- 1 year of LPN experience required
Credentials:
- Licensed LPN in State of Indiana
- Current driver's license
- Must have reliable transportation and insurance
