Summary of the Social Services Director
The ideal applicant will be passionate about helping others, while seeking fulfillment in a rewarding environment and strives to provide top of the line services to our aging population. The Social Services Director will ensure that the medically related emotional and social needs of the residents are met and maintained on an individual basis as it pertains to short- and long-term care residents.
Duties & Responsibilities of the Social Services Director
- Participate in discharge planning, development and implementation of social care plans and resident assessments.
- Interview residents and families to obtain social history.
- Involve the residents and families in planning social service programs when possible.
- Assist in arranging transportation when necessary.
- Refer residents and families to appropriate social service agencies when the Facility does not provide the services or needs of the resident.
- Provide information to residents and families as to Medicare and Medicaid, and other financial assistance programs available to the resident.
- Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
- Interpret social, psychological, and emotional needs of the resident and family to the medical staff, attending physician, and other resident care team members.
- Assist in obtaining resources from community social, health, and welfare agencies to meet the needs of the resident.
- Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.
- Assist in identifying, evaluating, and classifying routine and job-related social functions to ensure that tasks involving potential exposure to blood and body fluids are properly identified and recorded.
- Review departmental complaints and grievances from personnel and make written reports to the Administrator of action(s) taken. Follow Facility’s established procedures.
- Develop, implement, and maintain an ongoing quality assurance program for the Social Services Department.
- Participate in Facility surveys (inspections) made by authorized government agencies, as necessary or as may be directed.
- Ensure that all charted progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
- Involve the resident and family in planning objectives and goals for the resident.
- Meet with administration, medical and nursing staff, as well as other related departments in planning social service programs and activities.
Job Requirements of the Social Services Director
- Bachelor’s degree in social work, or bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology; AND
- One (1) year of supervised social work experience in a health care setting working directly with individuals.
- Effective verbal and written communication skills.
- Highest level of professionalism with the ability to maintain confidentiality.
- Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
