FRC Case Manager
The FRC Case Manager position is responsible for providing support and resources for families that qualify for case management services. The family in conjunction with the FRC Case Manager determine the areas they need assistance with and develop goals which are written out in a case plan. These goals are clearly stated on the Family Individual Service Plan (FISP). During the course of case management goals are met and updated according to the family’s progress.
Case management is offered to all families with children five years of age and younger. The Case Manager conducts home visits and meets with the families in their home environment. It is the goal to meet the needs of the family by linking them to resources within the community and assisting them in accessing these services. The FRC Case Manager also acts as a liaison within the community and is expected to develop relationships with partner agencies for the purpose of assisting families in overcoming barriers which prevent them from accessing services which ultimately improves the familial situation.
Case Manager duties include:
Computer skills in Word, Excel, and PowerPoint
Utilize Case Management assessment tools
Locate resources for clients and submit referrals for services
Network with partner agencies
Conduct community outreach via events, presentations, etc.
Facilitate groups as needed or assigned
Act as a liaison between clients and various social service agencies
Maintain timely and clear case note documentation
Conduct home visitation
Assist clients in accessing transportation to services
Translate for clients as needed
Assist clients in accessing basic needs (food, clothing, housing, etc)
Maintain flexible schedule for after-hours events/groups
Attend trainings, events at offsite locations
Other duties as assigned
Minimum Qualifications:
Bilingual-Spanish preferred
Bachelor’s degree in Social Work, Child Development, or related field preferred
AA in Social Work with 5 years of experience accepted in lieu of bachelors degree
Strong computer skills
Must have a vehicle
Valid CA Driver’s License/Insurance
Ability to pass a Criminal Background Check
Job Type: Full-time
Pay: $17.00 - $19.00 per hour
