This is a professional-level position designed to serve consumers at risk of psychiatric institutionalization or hospitalization, consumers who are on discharge status in institutions or short-term facilities, consumers who have been admitted or discharged from state hospitals within the past two years, and consumers with multiple short-term inpatient admissions. 

 

QUALIFICATION REQUIREMENTS:

  1. Bachelors degree in a mental health or related field.
  2. Individuals must have the ability to communicate effectively, both verbally and in writing with clients, in staffing meetings, and with supervisors.
  3. Knowledge of the local community resources is desirable.
  4. Ability to work a flexible schedule when necessary.
  5. Excellent organizational skills
  6. Ability to prioritize the needs of the clients.
  7. Able to work independently.

 

CORE COMPETENCIES:

  • Maturity and sensitivity to the special needs of PPBH clients while possessing knowledge of behavioral health and medical conditions. 
  • Possess the ability to document the services provided to customers in accordance with established governing standards in a timely, legible, accurate, and complete manner.
  • Ability to communicate client needs and information.
  • Ability to utilize and navigate an electronic health record and Microsoft Office software.
  • Must possess the ability to maintain client confidentiality

 

REQUIREMENTS: 

  • A valid Class E Drivers’ License for positions that require driving a personal or Agency vehicle on Agency business.
  • Must maintain a minimum Bodily Injury & Property Liability insurance limit of at least $50,000 for positions that require driving personal own vehicle (POV) on Agency business. (Proof Required)
  • PPBH is a 24/7 Agency.  All employees must be available as needed when required.

essential Duties and Responsibilities:

  • Assessment of high-risk consumers in terms of current and potential strengths, weaknesses and needs.
  • Develop holistic service plans with consumers addressing the broad needs characteristic of the service population.
  • Link and refer consumers to all internal and external services needed.
  • Ensure the proper documentation of progress notes related to requirements of Medicaid, licensing and accrediting agencies.
  • Coordinate cases to facilitate the implementation of the service plan.
  • Manage case records to assure that proper documentation is contained and meets applicable standards.
  • Initiate revisions and updates to reflect additions or changes in Medicaid, licensing and accrediting agencies.
  • Monitor the consumer’s progress with respect to the provisions of the service/treatment plan.
  • Evaluate the impact of services on consumers’ quality of life.
  • Advocate on the behalf of persons served to ensure that their needs are met.
  • Provide supportive crisis intervention to consumers when needed.
  • Other duties assigned by the Director of Case Management.

 

COMPUTER SKILLS: Must have Intermediate to advanced experience in MS Office Suite (Outlook, Word, Excel, PowerPoint), the use of personal computers, and standard office productivity software.

 

LANGUAGE SKILLS:

Ability to read and interpret documents such as safety rules, operation and maintenance instructions, and procedure manuals
