Job Title: Bilingual Licensed Social Worker/Case Manager

Overview:

Our organization seeks a dynamic and bilingual Licensed Social Worker/Mental Health Therapist to join our collaborative team. This role encompasses clinical and administrative responsibilities, providing psychotherapy and case management and contributing to the integrated behavioral health program. The ideal candidate will be fluent in English and Spanish, with at least two (2) years of clinical and administrative experience in a behavioral health setting.

Responsibilities:

1. Represent the Social Work Program within the department, collaborating with the Hidden Garden Program to contribute to patient-centered initiatives, policies, and evidence-based practices.

2. Perform assigned duties, including prevention, case management, assessment, and treatment planning.

3. Provide direct clinical services, including biopsychosocial intake assessments, diagnostic screenings, and individual, family, and group therapy.

4. Offer crisis intervention as needed, addressing emotional and behavioral health needs related to mental health issues, substance use disorders, physical health issues, and psychosocial stressors.

5. Document clinical encounters adhering to agency and licensing agency guidelines, supporting patient care and billing practices.

6. Adhere to departmental policies and procedures and comply with all applicable federal, state, county, and city laws and COMAR regulations.

Responsibilities:

1. Provide psychotherapy to HIV-positive individuals, couples, and families, maintaining a caseload of at least 15 billable service hours per week.

2. Deliver case management and crisis intervention services, connecting individuals to external services and evaluating ideation, housing, financial, and community resources.

3. Conduct biopsychosocial assessments and develop treatment plans using SMART goals in compliance with agency and COMAR regulations.

4. Screen patients for physical illness and analyze explanations for psychiatric illness, considering developmental issues, family dysfunction, genetics, neurological trauma, or social and cultural problems.

5. Collaborate with medical professionals as part of a multidisciplinary team to evaluate or adjust treatment plans.

6. Maintain confidentiality and patient privacy following NASW and HIPAA regulations.

7. Document care using patient-centered and trauma-informed language, meeting standards for quarterly chart reviews.

8. Seek consultations as needed and act as a consultant when requested.

9. Perform comprehensive assessments and refer patients for further care and services when necessary.

10. Navigate and document in an electronic medical record.

11. Present oneself with a professional demeanor and represent the organization/department positively.

12. Perform other duties as assigned.

Knowledge, Skills, and Abilities:

1. Current social work license in Maryland (LMSW, LCSW, or LCSW-C).

2. Clinical knowledge of factors associated with mental health and substance use disorders.

3. Ability to assess emotional, mental, physical, and social functioning using evidence-based tools.

4. Understanding clinical issues, ethics, values, standards, policies, and practices in an integrated behavioral health program.

5. Effective communication and collaboration within a multidisciplinary team.

6. Interpersonal skills to develop rapport and work with individuals/families with mental health disorders.

7. Knowledge of community resources and the ability to research and locate new resources.

8. Cultural sensitivity and knowledge of trauma-informed care.

9. Ability to work independently practice critical thinking, and problem-solving skills.

10. Skills in crisis intervention with flexibility and patience.
