Position Title: Case Manager
Job Summary:
Our Community Based Mentoring Program matches youth (mentees) with screened and trained adults (mentors) to build a quality and impactful relationship. Each youth mentee is carefully matched with an adult volunteer mentor that is recruited, screened, trained, and supervised by a Partners Case Manager.

This is a 1.0 full time equivalent, non-exempt position. Salary Range $48K-$53K DOE. Benefits include health insurance, retirement, PTO, 11 paid holidays, Christmas week off, wellness benefit, cell phone stipend. Reports to Mentoring Program Director.
Key Responsibilities
CASE MANAGEMENT
- Conduct interviewing and perform intake procedures on referred mentees
- Decide, with input from other staff, on appropriate matches of mentees and mentors & perform match meetings
- Build and maintain healthy relationship with a caseload of matches, unmatched mentees and pending mentors, parents/guardian, and referral agencies
- Conduct consistent and adequate case notes and records on all referred mentees, mentors and matches
- Conduct and support program youth evaluations
- Maintain healthy relationships and contact referral agencies to report on status and progress of referred and matched youth
- Develop and maintain a working knowledge of community resources and agencies, their function, and responsibility to most effectively aid and provide support to mentees and their families
PROGRAM SUPPORT with fellow Case Manager
- Plan, prepare and implement program activities for matched and waiting youth
- Implement mentor recruitment plan
- Implement communication plan for mentees, mentors and parents/guardians
ORGANIZATION SUCCESS
- Support the organization as needed with supplemental tasks (marketing social media, data, etc.)
- Work professionally with all staff as part of Partners for Youth’s integrated team and participate in related internal and external meetings as required
- Work within the context of Partners’ policies, procedures and guidance
- Promote the safety, welfare, and positive youth development of Partners youth participants
Required Qualifications:
- Excellent verbal and written communication skills, ability to relate effectively with people of varying life experiences
- Experience and/or passion for serving youth and families
- Ability to maintain confidentiality
- Ability to foster positive, supportive relationships with collaborative partners
- Must be willing to work occasional evenings and weekends
- Successful completion of Partners screening process
- Must have privately owned vehicle, current driver’s license and up to date insurance, as well as a willingness to drive a 12-passenger van as needed in varying road conditions
Desired Qualifications:
- Spanish Language skills knowledge encouraged; not required
- Bachelor’s or Associate’s degree (Social Work, Human Service or related field preferred)
- Knowledge of Routt & Moffat County, local schools and youth agencies
