Job Description:
This Clinician position is responsible for supporting the Program Administrator in conducting assessments, quality assurance, and the admissions process. This position will perform service coordinating and assessments of adults diagnosed with developmental disabilities. This position requires knowledge of the DBHDS, Human Rights and DMAS Regulations, fiscal and billing management, confidentiality and storage of records. supervisory skills, Knowledge of Individual's Rights, experience in behavior management techniques, Medication Management, and Supervisory training, as required by federal and state guidelines. Write, track and amend individual service plans for all individuals receiving services
Immediate Supervisor:
This position will report directly to the Program Administrator and Managing owners.
Responsibilities:
Services are provided in Noble Care's group homes and/or Day Services programs. The Clinician is required to develop and coordinate a PCP (Person Centered Plan), oversee the implementation of the ISP and service delivery.
1. Conducting intake and exit interviews, discusses program requirements with authorized representatives or guardians and placing agencies and make recommendations to the Program Director/owners whether or not to accept or discharge the individual.
2. Completes all required documentation to ensure Noble Care is in compliance with
DBHDS regulations.
3. Ensures that the Part five of the PCP ISP is completed, reviewed, being implemented, and coordinating with managers to ensure documentation is being properly completed by staff.
4. Maintains case management records. Review all individuals' documentations at least weekly. Utilizing documentation to compile information to prepare quarterly reports on individuals. Maintain contact with authorized representatives or guardians. Coordinate community support and intra-agency support as needed for each individual.
5. Complete the DSP (Direct Support Professional) Supervisory training, as well as any
other required trainings.
6. Ensure all annual consent forms and necessary yearly medical assessments (tb test, physicals etc) for the individual is completed.
7. Ability to work varied hours to oversee the implementation of programs and individuals.
8. Assist with developing and presenting the employee's monthly meetings.
9. Maintain continuing education by attending workshops, seminars and conferences.
10. Develop and revise forms for the program and agency services as needed.
11. Maintain professional ethical standards.
12. Prepares data for accurate billing for services, complying with the requirements of the varied funding agencies.
13. Adhere to all of Noble Cares Policy and Procedures.
14. File Individual's monthly documentation.
15. Any additional duties as assigned by the Program Director and/or Executive Director.
Qualifications:
1. Must possess a BA/BS degree in Human Services, Psychology, sociology, Social Work and have at least one year experience working with individuals with intellectual disabilities. Must have at least one year experience as a QDDP(Qualified Development Disability Professional)
2. .Have a valid driver's license
3. Approved Criminal Records Report; Criminal Background Check and Child Protective Services Background check
4. PPD Testing (TB)
5. Medication Training (At least a 32 hours training)
6. College transcript
7. CPR/First Aid Training
8. Must complete the DSP Supervisory Training, TOVA Training, Human Rights Training,
9. Strong knowledge of Microsoft Excel and Word preferred.
10. Strong Communication Skills a must.
