Overview
To assist with planning, implementing, and analyzing cultural and linguistic health education (C&LHE) services for PHC staff, Providers, and Members. Participates in state level C&LHE committee meetings and implementing work plans to meet regulatory requirements.
Responsibilities
- With direction, assists with Group Needs Assessment (GNA) research and reportpreparation for DHCS per regulatory requirements.
- Researches appropriate health education material for distribution to members.
- Provides education at community collaborative meetings and community events asassigned.
- Monitors and maintains information on both the provider and member portions of PHC'swebsite.
- Tracks and monitors all incentive requests ensuring all required documents are received ina timely manner meeting DHCS regulatory requirements.
- Establishes effective working relations with community resources and service agencies, aswell as other Health Plans. Acts as liaison for PHC on various community level and Statelevel committees.
- Assists in coordinating inter-departmental planning for internal C&LHE Workgroupmeetings.
- Attends C&LHE meetings, follows up on assigned tasks, and communicates the status ofprojects to the supervisor.
- Maintains department files for auditing purposes (hard and soft copies) per instruction.
- Monitors contractual, legal, and regulatory requirements for cultural and linguistic services
- Other duties as assigned.
Qualifications
Education and Experience
Bachelor’s Degree in Health Education, Public Health, CommunityHealth or related field; experience in Public Health Education, orequivalent combination of education and experience.
Special Skills, Licenses and Certifications
Certification in disease specific education preferred (I.E. DiabetesEducator; Asthma Educator). Valid California driver’s license and proofof current automobile insurance compliant with PHC policy are requiredto operate a vehicle and travel for company business.
Performance Based Competencies
Strong, effective oral and written communication skill. Ability to workwith ethnically and culturally diverse populations. Requires patience and ability to work with people of all levels of understanding and education.
Work Environment And Physical Demands
Ability to use a computer keyboard. More than 80% of work time is spent in front of a computer monitor. When required, ability to move, carry, or lift objects of varying size, weighing up to 25 lbs.
