Position Title: Intake Nurse
Reports to: Director of Admissions
FLSA: Non-exempt
POSITION: The Intake Nurse is responsible for modeling the three Company values of Compassion, Integrity, and Excellence, and for promoting the Hospice philosophy. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the Program. The Director of Admissions/Intake Coordinator assists in directing and managing the interdisciplinary team in the admission process the needs of the hospice patient and family in a manner consistent with the guidelines and policies of Passages Hospice. S/he communicates patient information to the business office and promotes a positive working relationship with the family, physician, and other agencies. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies.
PRINCIPAL ACCOUNTABILITIES * Office Nurse, faxes and calls prescriptions into the pharmacy. * Secures all documentation, processes, and deployment of resources (pharmacy, DME, etc.) for hospice patient admissions. * Ensures potential patients meet Medicare guidelines for admissions to hospice. * Obtains (IE) certification of terminal illness, physician order, history, physical, etc. * Oversees the communication of patient information, including but not limited to: admissions, deaths, respites, and changes in level of care via the Kinnser/WellSky system. * Assigns task upon admit. * Updates census board. * Scans documents.

Education and/or Experience
Licensed Practical Nurse with minimum of two years of experience. One of these years shall consist of full time experience in providing direct patient care in a hospice, home health, or oncology setting. Shall be available on-site or available at all times during operating hours and additionally, as needed.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, patients, colleagues, physicians and the general public.
Reasoning Ability
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables.
Computer Skills
To perform this job successfully an individual should be proficient in Microsoft Office and Electronic Medical Records software.
Other Qualifications
Understanding and acceptance of hospice philosophy. Must be of high integrity including maintenance of confidential information including all personnel and patient records. Excellent organizational and interpersonal skills are required. Ability to work a flexible schedule, to include some evenings and weekends. Must be able to travel.
Certifications, Licenses, Registrations
Must be a Licensed Practical Nurse licensed in the state of employment. Must have a valid driver’s license and auto liability insurance.
Other Skills and Abilities
Strong leadership, organizational and interpersonal skills. Must be able to read, write and speak English.
