POSITION ROLE
The Peer Recovery Navigator role is critical to Pathway Healthcare’s mission TO HELP PEOPLE. By role modeling Pathway Healthcare values, the Peer Recovery Navigator provides hope to individuals with addictions disease by identifying treatment options and facilitating the process of getting the individual the appropriate care.
ROLE RESPONSIBILITIES
- Establish and maintain relationships with key individuals in hospital ED and other departments.
- When requested, visit with patients in assigned hospital(s) that are seeking drug and/or alcohol addictions treatment and establish a relationship and provide the individual with information about Pathway Healthcare’s services and treatment.
- Facilitate and assist in scheduling the patient’s first office visit to meet with both medical and behavioral health professionals.
- Periodically follow-up with patients the Peer Recovery Associate referred to Pathway Healthcare and have been receiving treatment.
- When requested, provide new patients with an orientation to the office, sharing information related to the office’s policies and procedures.
QUALIFICATIONS & EDUCATION REQUIREMENTS
To be qualified, the Peer Recovery Associate must have:
- The life experience to allow the Peer Recovery Associate to empathize with individuals seeking treatment for addictions disease.
- A sincere interest in helping people get appropriate addictions disease care.
- The ability to establish and maintain relationships with health care providers, case managers, and other individuals working in acute care health care settings.
- Preferred experience working in a similar role.
- Certification is required
ENVIRONMENT AND PHYSICAL DEMANDS
- The Peer Recovery Associate works in a well-lit, climate-controlled office environment. The individual primarily works in a sitting position either facing patients or while operating a computer.
- The Peer Recovery Associate occasionally handles sealed, enclosed urine samples while wearing personal protective equipment.
- The Peer Recovery Associate occasionally must lift and carry up to 25 lbs.
