SUMMARY

The ACT Team Leader provides support to the Clinical Manager by ensuring that the daily operation is efficient, effective, and that quality services are rendered to consumers with complex severe and persistent mental illness. Ensures that services meet medically necessity, are trauma informed and are delivered using a person-centered approach. Leads and provides direct care services. The ACT Team Leader leads all efforts in responding to the emergent needs of consumers and tracks outcomes. Maintains strong positive relationships with key members of the funding source.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Participates in the direction of the day-to-day clinical operations of the Act team including scheduling staff work hours to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hours.
  • Provide direct clinical treatment, rehabilitation and support services to clients as necessary to include conducting individual, group or family therapy as needed.
  • Leads treatment planning and team meetings; continuously evaluates the status of clients and does appropriate planning and coordination. Assumes primary responsibility for developing, revising, implementing, and evaluation of treatment plans utilizing a person centered approach.
  • Monitors and ensures that the team delivers high-quality care that is consistent with FPS, DBH, and accreditation standards.
  • Consistently maintains fidelity to the model and assist in the attainment of key contract deliverables.
  • Leads, consults and provides crisis management and safety planning. Ensures the linkage of consumers to external processes that enhance quality of life indicators.
  • Participates in the training and orientation of new staff on the team. Participates in on-site coaching of staff on the team. Provides refresher/booster trainings as necessary. Collaborates with the Clinical Manager to develop, implement, and conduct ongoing professional development activities.
  • Develops and maintains key relationships with DBH, other external providers, and the community. Professionally represent the organization at required meetings, and other business-related functions.
  • Attend clinical supervision, training and other professional development activities as required.
  • Consistently maintain established productivity requirements and case assignments.
  • Performs other duties, as assigned.

TRAVEL

REQUIRED EDUCATION AND/OR EXPERIENCE

  • Master’s Degree in Psychology, Social Work, or a Human Services related field
  • Two years of direct clinical experience


PREFERRED EDUCATION AND EXPERIENCE

  • Experience in implementing MHRS regulations, policies and procedures.
  • Leadership experience preferred


CERTIFICATES, LICENSES, REGISTRATIONS

  • Clinical License in the state of Louisiana (LPC, LCSW) and adherent to all licensing board requirements to remain in good standing
  • Valid driver’s license


QUALIFICATIONS

  • Language Skills

Advanced clinical writing skills required. Ability to read, analyze, and interpret documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to articulate the needs of consumers in a concise manner.

  • Reasoning Ability

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Ability to problem solve and provide conflict resolution. Ability to develop needs assessments / root cause analysis techniques and strategies.
