Pathways Hospice has an immediate opening for a Chaplain.
Location: Ft. Collins, CO
Status: Full-time
Schedule: Monday-Friday, 40 hours
Pay Range: $54,662 - $61,609 annually
Supplemental Pay: Based on position, schedule and/or availability: Paid Mileage/Shift Diffs/Stipends
Mileage and Expense Reimbursement: Sixty-two and a half cents per mile – one of the highest in the industry!

REWARDING WORK YOU WILL DO:
Provides spiritual counseling and emotional support to patients and families/caregivers during the course of care and at the initial phase of bereavement. The position works collaboratively with the Interdisciplinary Team/Group (IDT/IDG) as well as with facilities and agencies in the community.
- Completes initial spiritual assessment. Develops spiritual care plan and provides spiritual care from admission to death or discharge.
- Schedules and visits patient and families to provide spiritual care and support. Provides education, direct spiritual counsel, prayer and ritual/sacramental ministry, as appropriate and requested, regarding the dying process and end of life issues.
- Acts as a liaison between the patient, family and their faith community when appropriate.
- Participates in IDT/IDG meetings to identify, evaluate and communicate feedback on the religious and/or spiritual needs of the patient and family as part of the Plan of Care (POC).
- Communicates risk indicators to Bereavement Services as appropriate.
- Assists with final arrangements, such as mortuary selection, planning memorial/funeral services, contacting clergy, sacraments/rites or other services according to patient and family goals. When requested, conducts sacraments/rites, worship, funeral or memorial services.
- Provides education about eye, body or tissue donation options. Helps facilitate donation process.
WHAT WE ARE GOING TO LOVE ABOUT YOU:
- Minimum Education: Master’s degree in Divinity or related field. One (1) unit of C.P.E required; two (2) units preferred.
- Minimum Experience: One or more years in a spiritual service role.
- Required License: Ordination by a religious organization or endorsement letter of good standing by a religious organization. Valid Colorado state driver’s license and proof of auto insurance. Current CPR certification based on individual affiliate requirements.
PHYSICAL REQUIREMENTS:
- Ability to lift/push/pull minimum of 30 lbs.
Pathways Hospice is affiliated with Care Synergy, a nonprofit network of community-based hospice and palliative care organizations serving Colorado’s Front Range and the Region’s Largest Home Health, Hospice, and Palliative Care Network. Care Synergy provides mission support services to better equip Pathways Hospice to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
