Description
Serves as the Initial point of contact behavior individuals seeking services and PVC. This position is vital in developing a strong engagement with individuals, including adults, children and adolescents. In order to complete the initial plan of care will be responsible for identifying the nature of the presenting problems and specific services, support the needs of individuals, planning advocating, and coordinating the delivery of needed services, including emergency services. May train others. Works under general supervision with moderate latitude for the use of initiative and independent judgment.
DUTIES & ESSENTIAL JOB FUNCTIONS
Under the supervision of the Intake Program Manager , provides an array continuity services assistance and support. By utilizing the information collected in the intake assessment will complete the initial plan of care, make appropriate referrals and link to additional services. Coordination with ongoing case manager will be necessary to ensure continuity of services and follow-up on referrals. Participates in the crisis on call rotation during weekend and evening hours.

Capacity for developing professional relationships with individuals requesting services as well as their families. Ability to articulate clearly and concisely orally and in writing. Ability to interact positively with both internal and external service
providers for purposes of referral and advocacy on behalf of the clients. Ability to learn about and access community resources available to meet needs of the clients. Flexibility in dealing with frequent changes and fluid situations common to the human services field. Must have computer operating skills. Ability to develop and maintain professional relationships with other staff. Ability to work independently, organize work, prioritize the needs of clients and of the agency, and use sound professional judgment.
Qualifications

Bachelor’s degree in a social, behavioral or human services field is required

Experience in a social, behavioral, or human services occupation is preferred.
- Must be able to work in a team setting. Knowledge of agency and community resources and continuity of service options available to individuals.
- Ability to be certified as a Qualified Mental Health Professional-Community Services.
- Must be at least 18 years of age
- A valid Texas driver’s license and good driving record is required along with a copy of a valid auto insurance card.
- Must be able to bend, stoop, kneel, crouch, squat, lift 25 pounds, sit for an extended period, and reach overhead.
Salary : $19 - $22
