Position Summary

We are seeking a highly skilled and experienced Administrator / RN Supervisor to join our Home Health Care agency. Acting in accordance with the Nurse Practice Act, as well as basic principles and best practices of professional nurses, a successful Administrator / RN Supervisor ensures that patients receive the best medical care possible by overseeing their well-trained team of nurses and aides. Their purpose is to maintain safety and patient confidence by ensuring all patients’ needs are addressed with the highest degree of professionalism and care, and that all policies and regulations are adhered to strictly. Further, this is a critical role that requires strong leadership abilities, excellent clinical knowledge, and effective communication skills.

Key responsibilities are to:

  • Maintain and manage a cohesive medical team.
  • Facilitate good communication between patients, nursing staff and physicians, demonstrating a passion to care for children and persons with disabilities.
  • Execute skilled assessments.
  • Develop care plans.
  • Promote continuity of safe, appropriate care through case management.
  • Manage financial statements and department budgets.
  • Lead/participate in corporate and regional strategic planning initiatives, then creating, executing, and successfully managing activities to achieve optimal levels of care and business growth goals.
  • Supervise assigned team members of RNs, LPNs, Home Health Aides, and Personal Care Assistants.

Qualifications

  • Graduate of an accredited school of professional nursing required.
  • Current license as a Registered Nurse in the state of Indiana required.
  • Baccalaureate degree preferred.
  • Minimum of one (1) year of nursing experience, plus at least (1) one year of supervisory or administrative experience in a health care setting, is required.
  • Home Health / Home Care experience is strongly preferred.
  • Previous DON or healthcare Administrator experience is preferred.
  • Effective computer, written and oral communication skills in addition to good interpersonal skills.
  • Proficiency in Microsoft Excel, Word, and Outlook required.
  • Proficiency using standard office technology and tools, such as a computer, copy / fax / scanner machines required.
  • Effective organizational and time management skills.
  • Has knowledge or the ability and willingness to obtain knowledge of home health regulatory and reimbursement requirements.
  • Current driver's license, safe driving record, and reliable transportation.

Essential Functions / Areas of Accountability

1. Reviews client referral information and responds to requests and inquiries as appropriate.

  • Verifies eligibility and evaluates appropriateness of client for home care services.
  • Determines appropriateness of referral for home care services and responds per agency policy and standard guidelines.

2. Performs client assessments based on agency standards of practice to ensure effective and appropriate home care services.

  • Applies standard nursing principles and practices to utilize a holistic approach to client care.
  • Considers the physical, psychological, and socioeconomic needs of the client, the level of family acceptance, and the ability for client needs to be met in the home.
  • Collaborates with physicians, other health care professionals (therapists, social services, pastoral care, supportive services and payor source case managers), clients, and families in developing a comprehensive, quality plan.

3. Directs, plans, and initiates appropriate action independently and responsively in home care situations as they occur.

  • Observes for and identifies overt and subtle signs of impending physiologic and psychological changes. Notifies the physician and payor source Case manager, Program Administrator and Interdisciplinary team in accordance with agency policies and procedures.
  • Makes decisions and/or recommendations that reflect consideration of immediate and long-range effects (frequency of services, components of care plan, additional care needs).
  • Confers with the Program Administrator on a regular basis. Utilizes agency and community resources appropriately.
  • Reviews documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness. Makes recommendations for improvements.
  • Communicates timely and effectively with other disciplines/departments as needed.
  • Travels throughout surrounding counties to serve clients, and provides them with on-call support. Occasionally, serves as back-up for RN Case Managers in adjacent northern and southern Indiana regions.

4. Manages/supervises a team of RNs, LPNs, Home Health Aides, and Personal Care Aides to provide effective and quality home care services.

  • Identifies the educational needs of team members and provides and recommends educational resources.
  • Monitors the performance of team members on a regular basis.
  • Identifies staffing needs and communicates this information to scheduling department.

5. Participates in human resource management to achieve quality service delivery and positive employee relations.

  • Assures that human resource policies and procedures are communicated to staff and are implemented in a fair and consistent manner.
  • Conducts timely performance evaluations consistent with agency policy. Conducts in-home supervision of staff per agency policy, regulatory requirements, and as needed.
  • Documents & applies disciplinary protocols in a fair and consistent manner when indicated per policy and within legal guidelines.
  • Monitors employee turnover, and absenteeism. Takes action to address and resolve issues.
  • Collaborates with Program Administrator to identify staffing needs and evaluate qualifications and competencies of current and new employees.

6. Promotes personal safety and a safe environment for clients and coworkers.

  • Demonstrates knowledge of safety infection control practices by compliance with policies and procedures.
  • Recognizes and responds appropriately to potentially unsafe situations.
  • Demonstrates safe and competent practice in the use of equipment.
  • Assesses safety of environment and takes initiative to prevent accidents and promote safety.
  • Participates in all mandatory education per established guidelines.

7. Performs job duties in accordance with agency policies, procedures, and professional and community standards.

  • Maintains confidentiality in all aspects of the job. Does not reveal information from client records to others, except as identified in agency policy.
  • Secures written confidential documents in a manner that prevents unauthorized release.
  • Participates in the development, implementation, and evaluation of the agency’s Quality Assurance and Performance Improvement programs and activities, and emergency disaster programs, to promote quality patient care and safety.
  • Provides care in accordance with state, federal, and licensure regulations.
  • Required to be on Indiana’s state Licensure and will serve as primary point of contact for all Indiana state surveys.

8. Performs other job duties as assigned.

  • Completes assigned tasks within established guidelines and time frames.
  • Familiarizes self with content of the Indiana Administrative code Article 17.
