QUALIFICATIONS:
Master’s degree in social services or behavioral health field and one (1) year experience working with children and families or Bachelor’s degree in social services or behavioral health field with at least two (2) years of experience working with children and families. Bilingual capability and community and school based settings preferred.
PHYSICAL REQUIREMENTS:
Ability to see and hear in emergency situations. Ability to communicate orally and in writing. Ability to travel independently to various sites within the community. Must be able to complete all portions of NAPPI training that are assigned and perform any NAPPI techniques for which the employee has been trained.
JOB FUNCTION:
Dynamic Fast Paced Case Management within a National Proto Type System of Care Wraparound service model. Provides assessment, development of the Family Support Plan, and linkage and referral to community based services and resources to assist in the alleviation of assigned families identified needs and/or service barriers. Facilitates a working partnership with the families, and effective collaboration with community providers to strengthen the overall sustainability of the Family and their functional connectedness long term with supports and resources.
SPECIAL WORKING CONDITIONS:
Exposure to youths and families served in a variety of program conditions, which may include flexible hours, various case management duties, collaboration with community partners, and local travel. Exposure to a wide range of community environments outside the agency. This position will serve 2 identified schools, and the PCS Truancy Court.

Responds to all telephone/electronic contacts, during assigned times, in a timely manner; responds to incoming referrals by contacting family within 2 business days to schedule an intake; assesses domains of need; develops effective Family Support Plan; completes a budget; provides community service and resource information and linkage appropriate to the identified needs of the family.

Completes all required documentation in a thorough, accurate, and timely manner: 1) Families are contacted within 2 business days; 2) Intake ppwk, Family Self Sufficiency Matrix assessment, and the Family Support Plan, and budget are reviewed at first visit to clarify domains of need and the role of the Navigator; 3) Progress Notes in Avatar are completed within one business day for any Navigation contact with or on behalf of a service recipient; 4) Required documents are scanned and attached to the client’s record in Avatar, the development and maintenance of the E record, and effective use of the E Signature by participant at the time of intake.

Provides Navigation/Case Management services for assigned caseload: 1) assists with assessment of appropriateness and effectiveness of services by conducting phone contacts and face to face visits with family; 2) maintains phone and/or face to face contact with provider agencies serving families to effectively collaborate for appropriate services; 3) advocates for and links family with other services and resources, both formal and informal, as indicated; 4) serves as coordinator of service network for family and involved providers; 5) takes any indicated action to reduce barriers to service utilization/access in circumstances in which service implementation has not occurred in a timely manner. 6.) works with collaborative partners for referral follow up, co location service delivery, and service coordination as indicated with PCSB and specifically the Transitional Zone Director and school social services; providing a weekly caseload report to school Administrators

Provides education in a family centered, youth guided approach for the family support plan and ensures that all information and referral data remains current: 1) provides updated information for team members’ community resource knowledge; 2) shares information verbally and or electronically.

Works cooperatively with Supervisors, Program Director, and PEMHS Accounting Dept. to determine need for Incidental Funds when indicated.
Works cooperatively with PEMHS Accounting to effectively disperse needed incidental funds when appropriate.
Provides back-up for team members Intake with data input required by PEMHS .

Completes all required basic orientation within the first three (3) months of employment.
Completes a minimum of thirty (30) hours of in-service training annually, including, but not limited to, training on the agency’s non-harassment policy, the agency’s Compliance Plan and Code of Conduct, NAPPI, CPR/First Aid, Infection Control, safety procedures, abuse reporting procedures, DCF HIPPA confidentiality, DCF DHH, Trauma Informed Care, Motivational Interviewing and Stages of Change.

Reports to Supervisor, Director, and/or Compliance Alert line any concerns regarding safety, potential abuse of individuals served by employees or agency, and regulatory requirements.
