Individual Supports Counselor - Full Time
Pay Rate: $18.00 / hour
$19.00 / hour (after one year of benefit eligible employment)
Schedule: Sunday 7am-7pm (Flexible)/ Monday -Thursday 1pm-8pm

Individual Supports
Our Individual Support Program is looking for compassionate people to serve as an IS Counselor providing premier services to the individuals in our care.
What does the job entail?
You supporting adults with development disabilities in Individuals' home. Individual Support Counselors implements individualized programs as specified in Individual Service Plans, provides for individuals health and welfare, and ensures general cleanliness and safety in individuals homes.
- Providing Individual community outings
- Encourage rapport with local resources
- Instruct and assist with meal preparation
- Assistance with activities of daily living
- Instruct and physically assist with housekeeping activities
- Medical appointments
- Administer and documentation of medication and/or monitor medical health
- Assist with financial management
- Ensure home is safe
What are Community Outings?
As a Direct Care professional you will provide opportunities for individuals to integrate into the community and enjoy outings that interest them. Examples of some outings are:
- The Movies
- Shopping
- Going to Dinner
- Baseball Game
- Bowling
