Title: Case Manager

Reports to: Program Director

Status: Non-Exempt (Hourly) – Full-Time – 32-40 hours

Job Summary:

Under the supervision of the Program Director, the Case Manager’s primary role is to work alongside families in Family Promise’s Emergency Shelter Program to acquire full-time employment, maintain savings, and permanent housing. The Case Manager will also work alongside the homeless services partner agencies in the Monmouth County area to assist and obtain resources for the families in our shelter-diversion and emergency shelter programs. In addition to working with the families currently in our program, the Case Manager will serve as a family advocate for alumni of the program for up to one year’s post-graduation.

Essential Job Responsibilities:

● Maintain an active and ongoing waitlist of potential candidates for our emergency shelter or diversion program to ensure complete utilization of the shelter’s capacity.

● Conduct prospective family screenings, intake, interviews, and selection in collaboration with the Program Director.

● Ensure family composition data are entered into The Homeless Family Information System (HMIS) upon intake and discharge from the shelter program.

● Assist Program Director in billing services through governmental and private funding sources.

● If and when the program is full the case manager will direct the families to other services and facilitate assistance through other organizations if applicable.

● Provide direct intensive case management and maintain accurate documentation for all families within the emergency shelter program.

● Responsible for dwelling visitation to ensure cleanliness in the shelter program.

● Coordinate with the Program Director in maintaining end of month reporting, including but not limited to families in the shelter program, clients assisted through diversion or outreach, and monthly highlights.

● Provide the following as a part of your case management plan:

o Life coaching, counseling, and mentoring.

o Monthly progress evaluations to determine room renewals.

o Budgeting, money management, and budget projection for housing and sustainability.

o Family relations support.

o Connect and mediate families with appropriate supportive services and housing resources.

o Refer all adults to the Life Skills Program and coordinate management of services between Case Manager and Coach.

o Maintain communication with the Program Director and motels in the area to obtain shelter placements.

● Determine family needs in the program, including post-graduate needs and help resource them adequately.

● Facilitate childcare resources and school enrollment alongside McKinney-Vento Liaisons.

● Ensure case notes and family files are up-to-date and complete, and uploaded to the secure shared case notes file.

● Responsible for coordinating family transportation and meal delivery based on location.

● Connect with graduate families for updates and to keep them connected with other Family Promise graduates, as well as provide life counseling and coaching to be resilient and maintain stability.

● Encourage graduates to continue being a part of the FP family circle, including paying forward through thoughtfulness/prayers, volunteering, and helping other graduate families.

● On-call duties for families in the program, such as communication outside of regular work hours.

● Other duties and special projects as assigned.

Requirements and Qualifications:

● Bachelor’s degree from an accredited college/university preferable in early childhood development, social work or related field OR 2 years of relevant experience in the field of homeless/human services.

● Experience or knowledge of the challenges associated with homelessness, including mental illness, substance abuse, violence, and isolation.

● Experience or knowledge of the principles and techniques of “beginning where someone is at,” engaging participants, establishing rapport, and building relationships.

● Experience of knowledge of the principles and techniques of advising participants regarding personal, economic, emotional, and social problems.

● Experience or knowledge utilizing trauma-informed care and de-escalation techniques.

● Knowledge of functions of community resources available to the participants.

● Knowledge of principles and techniques of preparing narrative reports.

● Knowledge of the local community of Monmouth County is a plus.

● Strong organizational and time management skills, and the ability to multitask, prioritize, and accomplish assigned tasks timely.

● Ability to positively interact with individuals while demonstrating sensitivity to socio-cultural diversity.

● Ability to maintain appropriate and professional boundaries.

● Proficient with Microsoft Office Suite (Word, Excel), Google Suite, and social media.

● Must be able to pass required background screening(s).

● A valid driver’s license and reliable access to a vehicle.

In addition to working in a typical office environment, the Case Manager may be required to work in areas inhabited by homeless persons, including working in unsuitable environments and dealing with challenged individuals.

Employees are required to:

● Walk and climb stairs.

● Reach with hands and arms.

● Be mobile by moving from place to place quickly.

● Carry and lift up to 25 lbs.

● Bend, stoop, or kneel.

● Employees will be subjected to outside environmental conditions while conducting various levels of outreach to homeless neighbors.

Job Type: Full-time

Pay: $22.00 - $25.00 per hour

Expected hours: 32 – 40 per week
