We are seeking a full time Central Admissions for our skilled nursing facilities around Hazelton, PA!
The Central Admissions Director will be responsible for managing patient referrals in order to ensure a smooth transition into one of our local nursing homes.
Responsibilities:
- Clinical assessment of electronic medical charts from hospitals to determine if a facility can meet the clinical needs.
- Verification of benefits
- Obtain pre-authorizations from insurance company
- Communicate clinical information to facility
- Ensure closure with each referral.
Qualifications:
- RN/LPN/LSW
or
- 2 years' experience in nursing home industry in one of the following roles:
- Central admissions
- Director of Admissions
- Clinical Liaison
- Director of Marketing
- Social Services
