What are the responsibilities and job description for the Director of Assessment & Referral position at Perimeter Healthcare?

The Director of Assessment & Referral is responsible for the telephonic and in-person risk assessment system, ensuring appropriate levels of care with a preliminary diagnosis, facilitating the initial treatment plan with the family, and implementing crisis intervention techniques. In addition, the Director of Assessment & Referral ensures rapid facilitation of admissions and maintains communication with referral sources, families, patients, and unit staff in order to have a consistent and efficient admission process.
Job Functions - Standards of Performance. Oversees the following:
  •  Admission process in its entirety of individuals after initial assessment.
  •  Orient patients and families to the treatment program.
  •  Adheres to facility standard policy and procedure.
  •  Promote an effective use of resources for patients, customers, and the facility with sensitivity to the cost of health care.
  • Related to adolescent/child patients in an age-appropriate manner.
  •  Maintains knowledge of program rules, protocols, and guidelines.
  •  Attends/participates in mandatory meetings and in-services.
  • Maintains confidentiality of facility employees and patient information.
  • Skill in prioritizing and organizing workloads.
  • Acts calmly in emergency situations.
  •  Provide information about the patient to the program staff who will be receiving the patient.

Specific Standards for the Care of Child Patients Ages 6 through 17. Oversees the following:
  •  Maintain knowledge of children's stages of development and provide guidance to all staff when questions about the norms for behavior related to child development arise.
  • Recognize acting out behavior of child-specific age groups and set appropriate
  •  limits in clear, concise, behavioral, and non-judgmental terms based on the age of the patient
  •  Relate to child patients in an age-appropriate manner.
  •  Utilizes behavioral management techniques with an understanding of the need for modification regarding smaller body sizes of children, abandonment issues, and the history of physical and/or sexual abuse issues.
 Director of Assessment & Referral
Specific Standards for the Care of Adolescent Patients Ages 12 through 18. Oversees the following:
  •  Maintain knowledge of the program rules, protocols, and level system.
  •  Recognize acting behavior of adolescents in specific age groups and set appropriate limits in clear, concise, behavioral, and non-judgmental terms based on the patient's age.
  •  Relate to adolescent patients in an age-appropriate manner.
  •  Utilizes behavioral management techniques with an understanding of the need for modification regarding smaller body sizes of adolescents, abandonment issues and the history of physical and/or sexual abuse issue.


Oversees the following - Performance Accountability:
  •  Uses time constructively and in an organized manner to accomplish duties/responsibilities
  •  Maintains a record of attendance/punctuality to meet expected standards.
  •  Participates in business development activities
  •  Complies with all facility, department, and corporate policies and procedures
  • Attends all mandatory facility in-services and staff development activities as scheduled.
  •  Adhere to facility standards concerning conduct, dress, attendance, and punctuality.
  •  Complies with Employee Code of Ethics/Professional Standards of Conduct
  •  Works as needed, including overtime, in case of emergency or as deemed necessary by supervisor.
  •  Promotes and demonstrates, through action, deed, and communication, an environment of service excellence throughout the facility, including internal and external clients, families, co-workers, physicians, and visitors.
  •  Demonstrates guest relations in communication, cooperation, and assistance for patients, families/physicians, licensed practitioners/co-workers, and visitors in a prompt and courteous manner.

Certification(s) / Registration(s) : CPI, HWC (not required)

Education Required: College degree
Type of Degree:
  • Master’s degree in Social Work
  • Professional Counseling
  • Associate degree in Nursing (required)
  •  Bachelor of Science in Nursing (preferred)
Amount of Experience:
  • Two (2) years of experience
Type of Experience:
  • Progressively responsible experience, preferably in a health care setting. Inpatient experience is preferred but not required.
