Job Title: Program Coordinator

FLSA Status: Non-Exempt
Availability: You must have open availability - Our shift starts as early as 8:00am and can last throughout the evening and weekends. If you have a hard stop time or a second job, this position may not be for you. However, if you are hard-working, can motivate a team, lead and produce results, this position is for you.
Location: Hybrid (Community, Office and Virtual - Must have a compatible laptop and mobile device
Summary: The Program Coordinator is to ensure continuity of care for the consumers. The Program Coordinator will be responsible for executing the growth action plan, also act as the liaison between the Support Coordinating Organization and the provider to ensure that all individuals receive the necessary supports and services they require and desire while apart of the agency’s program. You must be familiar with Charting the LifeCourse, In-Home and Community Supports, Community Participation Supports, Companion and Job Supports, Res hab along with their codes (6100, 6400, 2380). You must also have the ability to convert units and hours for budgeting. In addition, the Program Coordinator will be responsible for:

- Meet weekly expansion growth quota by way of referrals that actually select our organization as the provider by way of Meet & Greets, hold Caseload Review Meetings, outbound calls to targeted audiences using scripts, ensure Commitment packets by both the individual and family are signed on the call (upon entry to our program and annually), if we are selected, ensure all documents are completed by the individual and/or family members
Referrals: To understand and execute the vetting referral supports. We have a goal to support an additional 160 clients within the next 90 days
- Attend Meet & greets ensure confirmation of IHCS medical appointment dates are confirmed with updates and status, discuss communication plans, be extremely with final four, During Meet & greets ensure confirmation of IHCS medical appointment
- Participate in annual update and ISP revision, attending the ISP meetings, by providing last quarter progress notes and the next quarter, recommend vision to a service or outcome in the ISP, based on progress and/or lack of progress; therefore you will supervise, monitor and evaluate services provided to the individual
- Completing, signing and dating the monthly and quarterly documentation of an individual’s participation and progress toward outcomes.
- Complete the workflows in its entirety to ensure all steps are taken
- Complete daily, monthly, quarterly and annual status reports to include providing updates on your referrals that are in need of staffing
- Adding Authorizations and billing policy to Wellsky
- When position is vacancy and/or during down time, you make be asked to support other team projects such as recruiting and scheduling
- Vet ISPs to determine if PHCA can accommodate, do not deny supports to an individual, must get approval from upline before scheduling a Meet & Greet
- Reporting a change related to the individual’s needs to the SC, as applicable, and plan team members
- Coordinating the services provided to an individual by completing objectives
- Conducting Home and Community Visits
Program Planning & Compliance
- Facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, terminating, training, scheduling, conduct performance evaluations and oversight.
- Provide support to individuals receiving services including (you must be familiar with the ISP Manual and understand units, Day Program, IHCS, CPS 100% in the community and Companion and their codes.
- Ensure that the agency is in compliance with all QA & I guidelines
- Special Projects
- Conduct community events
- Conduct in-person all staff meetings and in-person lunch and learns
- HCSIS for ISP and PROMISe for eligibility
- Supports are being executed based on the ISP
- Conduct events hosted at TeamPHCA locations
- Collaborate with other Departments
Education/Experience:
Associate's Degree: Preferred
Language Ability:
Ability to read, analyze, and interpret general business periodicals, professional journals, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Experience: Must have experience with case load management, typing, reading, writing and interpreting emails, and motivating staff. We are especially looking for someone who has soft skills and can take positive approaches to resolving tasks. Positive attitude is a must.
