Job description

Primary representative to patients in the evaluation, prescription, trial, set-up, education, follow-up and modification of equipment requiring clinical training. Responsible for providing specialty equipment assessments ordered by a physician for patients of PHCS.

Supervision Received: Reports to clinical manager/ president/owner

Essential Functions:

1. Represent the organization to the patient and medical community in accordance with company’s mission.

2. Assessment, prescription, trial, set-up, education, modification and follow-up of all equipment requiring clinical expertise.

3. Assess patient and provide training listed on the patient’s Plan of Care.

4. Instruct patients during the initial set-up and follow-up visits on the use of the equipment ordered by their physician.

5. Evaluate the effectiveness and patient’s ability to properly use the prescribed piece of equipment and proper technique in using the prescribed pieces of equipment.

6. Clinical problem solve and follow-up with patients as identified on their Plan of Care.

7. Conduct follow-up calls with patients when required for purpose of continued education.

8. Contact physician and other referral sources regarding orders, complications or recommendations pertaining to their equipment.

9. Provide training to family and caregivers on equipment and services pertaining to equipment.

10. Participate in working business hours to assure all patients have access to care.

11. Review and monitor patient complaints.

12. Conduct reports on Patient Outcome data pertaining patients.

13. Participate and help improve the Quality Improvement programs of PHCS.

14. Conduct self in accordance with all of PHCS policies and procedures.

15. Adhere to best Standards of Practice guidelines.

16. Maintains strictest patient confidentiality and adhere to all HIPAA guidelines and regulations.

17. Participates in educational activities and attend staff meetings.

18. Participate in special projects as deemed necessary.
