JOB OVERVIEW:
The Public Health Nurse Home Visitor/Educator is responsible for providing comprehensive in home and community-based nursing services to pregnant persons and their families eligible for the Nurse-Family Partnership or the Mabel Morris Family Home Visit Program: An affiliate of Parents As Teachers (PAT). The Nurse Home Visitor/Educator maintains the highest standards in clinical practice and adheres to the NFP or PAT model. The Nurse Home Visitor/Educator upholds the policies, procedures, guidelines and standards of NFP, PAT and the lead agency and reports to the designated Supervisor and the Nurse Administrator regarding clinical practice and client/program outcomes. She/he, with the Supervisor, Administrator and senior staff, works collaboratively to build and improve the strengths-based client-centered culture. In this position, the PHN is also accountable to the Community Clinical Systems Integration (CCSI) initiative for 10% of their time.
RESPONSIBILITIES:
- Possesses educational foundation in Nurse-Family Partnership or Parents as Teachers program goals, theories, practice principles, rules of engagement and model elements through completion of all required education
- Supports the policies, procedures, guidelines and standards of NFP or PAT and the lead agency
- Participates in additional trainings such as breastfeeding, motivational interviewing, developmental and mental health screenings and other education as required to further enhance and support practice
- Contributes to professional development seminars and clinical conferences
- Adheres to nursing model of home visitation and community engagement
- Establishes and retains a caseload of pregnant persons, new parenting persons and eligible families in accordance with NFP and PAT requirements. Visits clients in their homes and/or mutually agreed upon location on a weekly or semi-weekly basis as dictated by model elements
- Develops therapeutic relationships with clients utilizing concepts of reflection and motivational interviewing
Supports, through one-on-one education, small group workshops and referral processes, the goals and achievements of NFP/MM parents and their families - Consults and collaborates with other professionals involved in providing services to parents and their families
- Plans home visits in accordance with client goals and program outcomes and documents all work appropriately
- Contributes to a positive work environment that enables all staff to function at their highest potential
- Meets weekly with supervisor for clinical supervision and utilizes reflective practice in supervisory sessions
- Schedules joint home visits with nurse supervisor
- Maintains confidentiality
- Participates in required regional, state and national program education sessions
- Collaborates with staff regarding community referral sources and disposition of referrals
- Engages in student learning work at undergraduate and graduate levels
- Assesses and documents physical, emotional, social and environmental needs of parents and their families. Records data on client needs, outcomes and milestones via computerized databases. Participates in regular review of reports and quarterly record audits for quality improvement purposes
- Supports the integration of primary care and public health nurse home visiting in order to improve patient care by creating consistent communication between public health nurse, families, and child’s pediatric provider
- Attend meetings with the Community Clinical Services Integration (CCSI) workgroup to discuss efficiencies and reduce burden on both PHN and pediatric provider, enhance coordination of care for families, and continue to identify areas of alignment in care delivery
- Implement strategies identified by the CCSI team with quality and participate in quality improvement efforts that aim to improve strategies for integrated care
QUALIFICATIONS:
- Current Pennsylvania nursing licensure in good standing
- Work or student experience in public health, maternal-child health, mental health/behavioral health or related field such as Early Childhood services in community settings. Plus is advanced education in motivational interviewing
- Strong interest in public health/home visitation/prevention services
- Detail oriented, in particular, timely data collection and entry
- Strong time management ability
- Excellent oral and written skills
- Moderate to advanced computer skills
- Collaborate with other professionals, families and community members
- Basic knowledge of community resources with interest in learning about relevant health, education, housing, social services and early childhood programs
- Strong interpersonal skills and the desire to work in collaborative public health model
- Interest in community advocacy/public policy work

- Has current Nursing Malpractice Insurance coverage
- If possesses automobile, valid driver’s license and current automobile insurance is required
- Active NPI number and eligible to apply and receive Commonwealth of PA Medical Assistance Provider number and bill as Provider 16 for child health services
- Has completed 2 step-TB test or pending
- Has completed drug screening or pending

ADDENDUM:
If selected for the 2022-2025 Fiscal Years Community Clinical Systems Integration (CCSI) initiative, the overview and responsibilities, in addition to the PHN model responsibilities, are outlined below.
The CCSI project is a strategic partnership between the National Nurse-Led Care Consortium and the Public Health Management Corporation with the Children’s Hospital of Philadelphia (CHOP) PolicyLab and the CHOP Primary Care Network to design and implement an integrated system of care between evidence-based home visiting services and pediatric primary care with enhanced support for family engagement in quality childcare. Through a shared staffing model, the Nurse Home Visitors will work with CHOP pediatric primary care providers to promote greater positive health care outcomes with clients/patients/families over time that address social determinants of health. Nurse home visitors bring unique and valuable public health expertise that this partnership will use to augment primary care services in defined locations. Alignment and coordination of preventive services delivered in the home and office settings will increase efficiency and quality of care for children served by nurse home visitors and pediatric primary care providers.
Public Health Nurse and Integration Specialist
The work pattern is altered for this initiative. This is a FTE position funded .9 by Philadelphia NFP/MM and .1 by CHOP/Vanguard with .90 FTE nurse home visitor serving clients as determined by the evidence-based model and .10 within integrative care efforts. The .10 FTE will be used to support the continued implementation of the identified integration strategies with additional responsibilities to be determined in collaboration with NFP/MM nursing supervisor(s), CHOP designated nurse manager(s) and project planning team to support the role of ambulatory care and the goals of NFP/MM to meet pilot project determined deliverables. The following deliverables/responsibilities for this integration project are detailed below and are complimentary of the PHN Nurse Home Visitor responsibilities detailed above.
- Establish and retain a caseload consistent with model expectations for the designated FTE. Evidence-based models weekly or semi-weekly visits with clients
- Serve as a core member of the integration planning team which has the responsibility to design, implement and evaluate the success of the piloted integration model
- Serve as a member of clinical care team for the CHOP Care Network pilot site, including care team communication, referral coordination, and other clinical responsibilities to be determined by the integration planning team
- Ability to navigate and overcome unanticipated challenges and quickly adapt to new circumstances and changes as they arise within the scope of the CCSI effort
- Proficient in basic data requirements by program, state data base, and receive training to be proficient in EPIC data entry
- Contributes to the ongoing planning processes that frames a written document outlining alignment with evidence-based home visitation model and pediatric primary care providers and enrolled client/families
- Determines with others quality improvement initiatives that will be evaluated over time
- Adheres to all CHOP educational requirements including annual safety protocols and updates to patient electronic medical record system (EPIC)
- Documents alignment and services including parent/child health and development screenings as determined by the integration team - such as Ages and Stages, using EPIC
- Collaboratively works with PolicyLab staff, primary care providers, nursing clinicians and NFP/Mabel Morris colleagues, supervisors and others as designated to develop a positive, productive and reciprocal communication strategies that support family health, engage providers clinical conferences that highlight the collaborative nature of program integration
- Share lessons learned and program outcomes with colleagues, interested organizations and providers at local-national conferences
- Communicate with the PolicyLab CCSI project director when necessary
- Maintain close communication with the Team Supervisor at all times
- Function as a CHOP/NFP/MM liaison with individual teams as well as all staff as needed
EXPERIENCE: NURSE ELIGIBILITY FOR NNCC/PHMC/CHOP INTEGRATION PROJECT:
- Has implemented at least one-year of the Nurse-Family Partnership curriculum or preferred to have completed at least one year of Parents as Teachers program curriculum
- Nurse-Family Partnership nurse is certified in DANCE and one or more re-certifications. Mabel Morris nurse is proficient in the implementation of PICCOLO assessment
- Proficient in or gaining motivational interviewing skills
- Participated in one or more NFP or PAT State meetings with accompanying Continuing Education Credits (CEUs)
- Contributes to at least one internal committee such as LATCH, TransFORMers, Changents (quality improvement), Advisory Council, Nursing Workforce, Graduation, Annual Community Action Day, Community/family CPR, Group Connections
- Commitment to the term of the project with one-year renewals based on on-going contributions to the project
