PURPOSE OF THE POSITION:

The Program Specialist will assist the residents at New Hope and other consumers of Phoenix Community Development Services programming by providing and linking them to a continuum of supportive services. The staff will also help ensure the safety and security of the residents, as well as the safety and security of the facility. In addition to those responsibilities, the Program Specialist will perform additional administrative duties. Applicants are encouraged to possess time management, organization, and conflict resolution skills.

SCOPE OF DUTIES AND RESPONSIBILITIES:

Provide screening and assessment to ensure consumer access to necessary and culturally appropriate services while ensuring that Phoenix CDS policies and systems governing screening and assessment are implemented and evaluated.

  • Knowledge of program requirements and screening tools to determine eligibility for PHOENIX CDS, PATH, HUD, and ESG homeless, mental health, and/or housing services.
  • Use Phoenix CDS criteria and screening tools to determine if consumer should be screened for SSOC mental health and/or housing services and report screening results to appropriate staff
  • Articulate screening, admission, and program criteria to consumers, external referral sources, family members, etc., to ensure understanding of screening, admission, and program decisions
  • Comply with confidentiality regulations and obtain consent from the consumer.
  • Determine if consumer should proceed to assessment based on established Phoenix CDS screening and admission criteria
  • Has general knowledge of coping skills appropriate for depression and for anxiety
  • The ability to recognize the warning signs associated with suicide and how to respond.
  • Consults supervisor when concerned about a consumer’s mental health
  • The ability to demonstrate effective listening and crisis management skills in day-to-day interactions
  • Effectively listen and establish rapport with consumers and families, and compassionately understand/respect differences.
  • Maintain appropriate treatment boundaries with residents and their families/visitors.
  • Document and report to appropriate systems and personnel
  • Support facility level community activity coordination; lending resource to events, access needs and equipment (on demand)
  • Conduct meetings as scheduled and appropriate and maintains relevant minutes/documentation. Completes operational requirements by scheduling, assigning, and completing administrative projects and reporting
  • Respond promptly and professionally to incoming phone calls and visitors

Help ensure the safety and security of the residents as well as the safety and security of the facility

  • Learn emergency and non-emergency procedures specific to the facility and be able to implement and facilitate emergency operations safety drills as directed
  • Responds to emergency situations in a calm and decisive manner and according to agency policy and communicates with appropriate agency contacts or other emergency services
  • Address security and other concerns/issues with consumers in a respectful and non-confrontational manner
  • Complete timely and accurate documentation regarding significant events on each shift worked
  • Follow mandatory safety and infection control procedures and guidelines and uses prescribed safety equipment as needed
  • Recognizes and reports all potential safety and maintenance issues to appropriate staff within the required time frame
  • Performs housekeeping tasks as needed in front office space
  • Completes all consumer charting/daily logs, data entry, incident reports by end of shift.
  • Maintain supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies.

Perform other duties as required and assigned. This includes but is not limited to:

  • Work effectively with consumers one-on-one or in groups
  • Develop appropriate clinical relationships with consumer of different cultures, races religions, sexual orientation and gender
  • Communicate effectively verbally and in writing
  • Represent Phoenix CDS in a positive and professional manner
  • Identify consumer needs and refer to appropriate ancillary service providers
  • Use Rules of Professional Ethics, Regulations and Confidentiality and apply to daily work situations
  • Make appropriate clinical decisions based on Phoenix CDS criteria

QUALIFICATIONS/SKILLS/COMPETENCIES

Must have the following:

  • Current and valid driver’s license
  • High School Diploma or GED
  • Must have the ability to work constructively with residents, treatment resources, and community agencies/resources

Preferred Qualifications:

  • Experience in a field related to mental health supportive services or serving individuals experiencing homelessness
  • Communicate effectively verbally and in writing.
  • Knowledge of various clinical criteria related to issues confronting our consumers (i.e. mental health, substance abuse, housing)
  • Knowledge of local service provider network (i.e. substance abuse treatment, mental health services, employment readiness services)
  • Use Rules of Professional Ethics, Regulations and Confidentiality and apply to daily work situations.
  • Basic knowledge of Windows office systems such as Word, Excel and Outlook

WORK CONDITIONS

  • Work primarily includes supportive residential setting, but may also occasionally include office and community settings
  • Requires ability to work in noisy and crowded environment.
  • Expect to interact with staff and consumers to provide activities related to work such as conducting meetings as well as workshops
  • Requires prolonged sitting or standing and may require prolonged walking or climbing stairs

Job Type: Full-time

Pay: $15.00 - $16.00 per hour
