Personal Assistance Coordinator

REQUIREMENTS:

• Must be 21 years of age or older
• Must have valid driver’s license
• Must have worked at least 2 years work experience with persons with a developmental disability or persons with mental health disability or the person may have post high school educational training in these areas.
• Must have computer experience, especially in Microsoft Word and Excel
• Must have the ability to interpret and instruct staff in the implementation of all plans: ISP, Behavior Plan, Health Risk Management Plan, Medication Administration, and Seizure Management Plan.
• Demonstrate the ability to communicate effectively with regard to completing all required documentation and reports and following all written and oral instructions.
• Demonstrates knowledge of all Agency policies, procedures and State/Federal Regulations.
• Demonstrates ability to consistently ensure staff compliance with all Agency policies, procedures, and State/Federal Regulations
• Must have completed all areas of required training with a competency score of 80% or above.
• Demonstrates ability to train staff in required agency training to include but not limited to PCISP, Behavioral Programs, and Healthcare Plans.
• Demonstrates skills to perform supervisory tasks according to Agency policy/procedures and State/Federal Regulations.
• Must have CPR training approved by the American Heart Association
• Must have a negative TB or chest x-ray.
• Must meet all requirements of the Criminal Background check

Specific Responsibilities:

· Coordinates with case managers that the consumer’s Person Center Individual Support Plan needs are being met

· Meet with potential clients for PAC/CHIO services

· Complete intakes on potential PAC/CHIO clients

· Build PAC/CHIO plan for clients

· Hire and train staff for PAC/CHIO clients

· Build PAC/CHIO client schedule for staff

· Observe staff during PAC/CHIO during schedule times

· Complete billing and payroll duties for your caseload

· Review and file accident and incident reports and forward to QIDP or Administration

· Communicate with QIDP, Administration, & Nurse.

· Assist with human resources for employees

· Other duties as assigned by the administration

Job Type: Full-time

Pay: From $16.00 per hour
