Phoenix Preferred Care is hiring Targeted Case Manager for our Louisville metro office.
Qualified candidates must have a Bachelors degree in a human services field AND one year of experience working with children and families following graduation.
The position includes providing targeted case management services to clients of a variety of populations, including children, adolescents, adults, and families. This involves conducting needs assessments, coordinating appointments, linking clients and families to community resources, and facilitating service team meetings.
