POSITION PURPOSE:

To provide overall direction, training, management, and leadership of Ramsey County DD/Rule 185 and CCB case managers who provide coordination of services for vulnerable individuals and assist them in navigating complex service delivery systems. This position directly oversees the Ramsey County Case Management division. The position requires extensive writing, communication, organization, and management skills; effective decision making, and problem-solving skills are also critical; working with various team members and excellent public relation skills are also required. Creativity and follow through on delegated tasks are required. Ability to evaluate case managers and make recommendations to address issues through the development of Best Practices is required.

NATURE AND SCOPE:

The Case Management Supervisor reports to the Supervisor of Case Management Services who reports to the Executive Vice President who reports to the President. The Case Management Supervisor is responsible for a variety of individual tasks, which require knowledge of disabilities and related conditions. Excellent interpersonal relationship and leadership skills are required. The job requires frequent contacts with clients, families, guardians, service providers, legal representatives, and other members of the interdisciplinary team to coordinate services for vulnerable individuals and assisting them in navigating the complex service delivery systems. The job requires frequent communication with the Lead Agency (Ramsey County) and the Department of Human Services. The job requires frequent contact with case managers including active mentoring, correction of work, and verifying that internal processes/guides are accurate. The work environment requires frequent standing, sitting, and occasional walking. Valid driver's license and ability to drive is required. The Case Management Supervisor participates in frequent phone, desk work and computer work.

Essential Functions of the Job- Case Management Supervisor:

  • Providing overall leadership of Ramsey County Case Managers. This includes completing annual evaluations, monitoring quality of work, assisting in creative thinking, and updating guides/Best Practices documents. This is assisted by the Ramsey County Case Manager Mentor and Supervisor of Case Management Services.
  • Providing overall direction on program regulations, either from Department of Human Services or Ramsey County; maintaining and building knowledge on rules/policies.
  • Manage incoming referrals of individuals needing services from the Lead Agency (Ramsey County). Includes tracking incoming referrals, assigning them to a case manager, maintenance of a list of available case managers, and tracking overall caseload sizes.
  • Budget management. This includes monitoring and evaluating case managers based on overall efficiency scores, tracking quarterly bonus structure, and managing costs in an efficient manner.
  • Leading and coordinating case management team meetings with assistance from Supervisor of Case Management Services.
  • Review and make final approval of Consumer Directed Community Support (CDCS) plans with assistance from case managers.
  • Reviews of internal audits done by case managers with assistance from the case manager mentor.
  • Maintaining a strongly reduced caseload of individuals receiving services (around 5-10 individuals at any one time). This may include providing coverage for absent case managers. This will result in the below essential functions:
  • Assists clients in locating and gaining access to necessary services as well as medical, social, educational, or other needed services regardless of the funding source.
  • Ensures informed choice for all individuals.
  • Coordinates person centered plan of care using person centered principles that balances what is important to a person with what is important for a person based on their needs and preferences.
  • Monitors services so the client's health and safety are assured.
  • Conducts face to face visits every six (6) months or as needed.
  • Assists in the identification of potential service providers and plans for/arranges services and informs county waiver contact of proposed waiver services to be authorized, for entry into MMIS.
  • Responds to all Minnesota Adult Abuse Reporting Center (MAARC) reports in a timely manner of receipt of report, documenting all responses and available for review upon request.
  • Sits and writes and/or types frequently in order to prepare reports, forms, or information related to overall plan of care.

Functions as a role model, and communicates effectively with clients, families, co-workers, and outside agencies, verbally and in written format; listens accurately and responds appropriately.

  • Ability to work with clients with a variety of cultural, medical, psychiatric, and physical needs.
  • Displays positive, supportive, and cooperative conduct. Maintains and fosters cordial relationships and optimal communication.
  • Adheres to and maintains knowledge of various rules, regulations, and state and county policies regarding the CADI, DD and BI waiver programs, Rule 185 Targeted Case Management; adheres to all policies and procedures/rules and regulations of Phoenix Service Corporation and its governing agencies.
  • Completes all necessary documentation. Develops systems that provide continual improvement in quality services.
  • Monitors and responds to emergencies according to established policies and guidelines; maintains an injury free workplace by identifying and reporting unsafe working conditions.
  • Participates in on-going education in topics regarding disability case management services; attends and completes all annual in-service requirements.
  • Identifies and responds to client needs in emotional and social areas, however, intervention in these areas does not fall within the meaning of psychotherapy.

QUALIFICATIONS:

  • Must have waiver case management experience (Hennepin or Ramsey County MN)
  • Public Health Nurse, Registered Nurse or Graduate of an accredited four-year college with a major in social work, psychology, sociology, or a closely related field; OR a graduate of an accredited four year college with a major in any field and 2 years' experience as a social worker in a public or private social service agency
  • 1-2 years experience with SSIS and MNChoices systems required
  • Overall leadership experience of 1-2 years preferred
  • Excellent communication and interpersonal skills
  • Excellent problem solving and organizational skills
  • Telephone, computer, and communication skills
  • Ability to read, write and speak the English Language in order to adequately interact and respond to the individuals and meet their service needs.
  • Ability to use a computer for required documentation, communication, and time log purposes.
  • Ability to use Microsoft Office products including Outlook, Excel, Word, and PowerPoint
  • Ability to see and hear adequately to respond to emergency situations according to established Phoenix Service Corporation policies and procedures.
  • Ability to travel to meet with clients and attend meetings, and if driving, holds a valid drivers license and maintains a safe driving record.
  • Ability to sit, stand, reach, and bend.
