Position Summary: The Patient Care Coordinator works under the general direction of the Practice Administrator and is a member of a dynamic team focused on coordinating all aspects of patient care. The role provides continuity of care by ensuring a smooth transition between care settings and by developing a trusted relationship with patients to identify and assist with each patient’s individual needs that may arise related to the care they receive.
Key Duties and Responsibilities
Patient Access
- Perform opening, closing and the completion of daily office duties
- Accurately schedules and coordinates patients’ appointments
- Calendar management, ensuring accuracy and follow up as appropriate
- Answer the telephone in a professional and courteous manner and directs calls appropriately
- Uses effective oral and written communication with a variety of Health Care Professionals, patients and their advocates, hospital and clinic personnel both at the facility and other medical facilities, representatives of insurance companies and other organizations related to the health care industry
Coordination
- Act as a liaison between patients, consulting physician, referring physician or outside agency to coordinate patient appointments and/or care. Will coordinate the exchange of information either written or verbal before and after patients are seen
- Triage calls from referring physicians, patients, family members, and others wishing to access care
- Maintain completion and accuracy of all PCC touch points in the patient chart, including Demographics, Contacts, Notes, Portal, Documents, ect.
- Facilitate the flow and accuracy of documents between patient, providers and payers
- Assist with the financial clearance process as necessary
- Receive and respond to provider/patient feedback and requests as appropriate
- Actively participates in departmental and/or organizational process improvement initiatives
- Assists with ancillary duties as assigned by the Practice Administrator
- Maintain HIPAA compliance

Qualifications
- AA degree in healthcare related field or the equivalent
- One (1) year of experience in a fast paced, multifaceted environment
- Knowledge of exceptional customer service concepts and techniques
- Proficient in the use of computers
Preferred Qualifications
- Electronic Medical Records (EMR) experience
- One (1) year experience in medical/healthcare setting or two (2) years in multifaceted, fast paced customer service role
- Basic knowledge of HIPAA and ability to maintain compliance
Physical Requirements
- Sit for long periods of time
- Lift 10-15 lbs
- Spend long periods of time at computer
Location:
We have two locations (Columbus & Dublin). This role will be split between our Columbus and Dublin locations and will be expected to float as needed.
Hours:
We are open: M-TH 8am-4:30p and Fridays 8a-1p
We are open to bringing on a Per Diem Patient Care Coordinator in the event we need some ancillary assistance throughout the week and/or in the event of call offs or PTO/vacation requests.

Hours are flexible throughout the week based on clinic operations and with approval of the Practice Administrator.
