Pinnacle Wound Management is currently seeking a Nurse Practitioner to join our network. Our partnered specialist visit the patient at the nursing home and develop comprehensive plan of care with the nursing staff.

Duties/Responsibilities:
- Travel to each assigned nursing facility weekly
- Assess patient and make recommendations on plan of care
- Work closely with nursing staff and facility administration
- Utilize facility formulary and follow facility protocols
- Document patient encounter in our EHR to ensure timely reporting
- Discuss treatments and goals with PCP and family members
- Procedures – Debridement’s, I & D’s, Cauterization, NPWT, etc.

Requirements:
- Unrestricted NP License in state of practice
- DEA
- Must pass state and federal background checks
- Basic EHR knowledge
- Team Player
- Strong Communication skills
- 1 year wound care experience
- Wound Care Certification (preferred)
