What are the responsibilities and job description for the Case Manager position at Piper Companies?
Piper Clinical Solutions is currently seeking a Case Manager in the area of Lanham, MD. The Case Manager will be responsible for supporting a medical center that specializes in mental and behavioral health, counseling, crisis intervention, and ensuring compassionate care.
Responsibilities for the Case Manager:
- Identify and Prioritize patients in need of care management services, and assess care plans to include any cultural, spiritual, biopsychosocial, functional, or financial factors
- Ability to conduct motivational interviewing in order to promote patient engagement, treatment adherence, self-care, and overall health and well-being.
- Utilize InterQual or other evidence based guidelines to promote quality of care, decrease variation, and mitigate waste
- Utilize EPIC system to enter clinical reviews, documents, and patient records and refer to Physician Advisor as needed
- Enroll patients in appropriate transitional programs such as home health, community care coordination program, Hospice/Palliative care, routine sub-acute or skilled post-acute providers
- Ensure all documentation/patient records are accurate, clear, and concise and prepared for necessary handovers (should reflect all patient needs/care plan/financial info/etc)
Qualifications for the Case Manager:
- Experience as a care coordinator preferred
- Familiarity with EPIC system preferred
- 3 (three) years of experience in an ambulatory, post-acute, or clinical setting
- An active license as a registered nurse certified by Maryland Board of Nursing
- Bachelor of Science in Nursing
Compensation for the Case Manager:
- Salary range: $62,000 - $93,000 (commensurate with relevant experience)
