General Description
The Quality Improvement Coordinator is responsible for supporting and championing the Performance Improvement processes/philosophy/activities throughout the agency.
Job responsibilities:
- Promote culture of continuous quality improvement
- Work with teams/departments to identify appropriate performance improvement models, with both clinical and non-clinical staff, to address gaps between actual and target performance.
- Monitor and tracks CCBHC quality indicators, HCH indicators and PfP indicators and works with teams/departments to identify performance improvement opportunities.
- Assist with communicating to CQI Committee any over arching agency barriers to implementing identified improvement plans.
- Monitor any improvement plans in place and reports on progress
- Analyze information and data systems to evaluate their performance, formulating ways of enhancing workflows
- Conducts analysis of data in cooperation with staff to assist in identification of improvement activities
- Keeps Directors/Managers informed of current KPM’s results via a Dashboards and Scorecard, and any areas that may require education for KPM’s or new KPM’s.
- Conduct monthly internal administrative and clinical audits.
- Develop clear workflows when needed and provide training on clinical and business workflows to teams and departments.
- Assist with any policy review around agency quality improvement
- Support the Risk Committee by gathering any requested data to ensure optimal risk mitigation.
- Assist team assistants in clear direction on indexing, storage or disposal of records in electronic health records
- Responsible for maintaining appropriate databases and data entry into those databases in regard to performance improvement activities
- Work with the EMR and Evaluation/Research departments toward the integration and synthesis of clinical and administrative quality data, utilization data, outcome data and cost data
- Assist with agency certification process with preparing needed documentation
- Complete annual training requirements as outlined by agency requirements and supervisor
- Performs other related tasks as assigned by the supervisor
Requirements
- Bachelor’s Degree, Master’s preferred.
- Basic familiarity with appropriate computer software (Microsoft Suite, database, etc.).
- Mastery of basic requirements of medical records organization and management.
- Ability to maintain effective working relationships with all levels of staff and clients.
- Self-directed and able to work effectively without close supervision.
- Able to maintain a high level of confidentiality with regard to client and agency information.
