Position: Supportive Living Program Case Manager Location: Dayton-(Admin), OH Job Id: 292 # of Openings: 1
SLP Case Manager
PLACES is a community mental health not-for-profit provider that has been in business for 35 years focusing on providing housing, CPST, and supportive services to those diagnosed with a behavioral health disorder/ mental illness and homelessness. PLACES focuses on quality of life and community integration. Supportive Living Program Qualified Mental Health Specialist ( CPST) will provide Community Psychiatric Supportive Treatment(CPST) services, case management, and life skills based on Behavioral Health Service Plan (BHSP) to adults diagnosed with behavioral health disorders, including those experiencing homelessness to aid their ability to integrate into the community. This CPST, community psychiatric supportive treatment position is M-F, 8:30-5p. The candidate should have case management experience and the ability to relate to the homeless and mental health population.
Minimum Qualifications
Preferred Qualifications
High School Diploma or GED
Associates degree or higher
One year of direct service or related experience in the field of social services
Three years of experience in behavioral health client care, including the homeless
Valid driver's license
Case management/QMHS experience
Insured personal vehicle
Knowledge of community resources
Good driving record
Knowledge of Motivational
Insurable under PLACES auto insurance
Interviewing, Stages of Change,
Demonstrated competency in basic
Trauma-Informed Care, and Harm
computer applications
Reduction models or other accepted evidence-based practice
Demonstrated competency in clinical documentation meeting Medicaid & managed care standards
