This is a field-based position that requires daily travel to the member’s place of residence. In the home, the provider will review and interpret/verify the member’s medical and social history in addition to appropriate medical screenings.
Qualifications
- Current, unrestricted NP license in state of practice
- Current BLS certification
- Previous experience performing in-home assessments preferred
- National board certification in adult, family or geriatric specialties
- Must have valid state driver license and automobile insurance
