Registered Nurse – Special Needs Coordinator

Position Duties and Responsibilities:

  • Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness. 
  • Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF. 
  • Perform physical exam and health histories. 
  • Provide health promotions, counseling, and education. 
  • Administer medications, wound care, and numerous other personalized interventions. 
  • Direct and supervise care provide by other healthcare professionals. 
  • Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel. 
  • Recognize adverse signs and symptoms and quickly react in emergency situations. 
  • Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization. 
  • Make referral appointments and arrange specialty care as appropriate. 
  • Perform nursing services identified in the TO. 
  • Conduct research in support of improved practice and patient outcomes. 
  • Participates in evidence base practice and clinical support protocols. 
  • The SNC registered nurse oversees and manages the installation Exceptional Family Member Program-Medical (EFMP-M) IAW DoD and AF policy, and any subsequent implementing guidance. 
  • Uses clinical assessment skills and maintains procedures to identify sponsors whose family members have special medical and educational needs in a timely manner. 
  • The SNC uses a multi-disciplinary and collaborative approach with other key service providers, such as installation youth and childcare facilities, and officer and enlisted spouses’ groups, to ensure effective outreach and identification of special needs. 
  • Ensures family members of the AF personnel or family of the AF member are appropriately “Q coded” (EFMP-enrolled). 
  • Ensures all active-duty sponsors known to the Air Force Personnel Center, and local Military Personnel Section and/or Commander Support staff where applicable, as having family members with special needs are identified in AF medical special needs data management systems. Provides oversight for base-level data entry in AF-provided data management systems used in the management of EFMP-M. 
  • Provides clinical training to installation personnel and medical staff; and consultation as needed to support the implementation of EFMP-M base-wide. 
  • Integrally involved in the Family Member Relocation Clearance (FMRC) process. 
  • Ensures all FMRC requirements are implemented by all EFMP-M staff. Coordinates the enrollment process and travel screening for all active-duty family members’ and travel screening upon request to the families of DoD sponsored assignments going to an overseas base. 
  • Provides clinical oversight to ensure every AF sponsor with one or more family members with special needs assigned to the installation has a Special Needs file maintained at the Medical Treatment Facility (MTF). 
  • Ensures Q-coded sponsors assigned to the installation are contacted annually to determine if there are unmet needs and to request updates of information as needed. 
  • Collaborates with the Integrated Delivery System (IDS) and other interagency forums that provide family medical information and referrals to base and civilian agencies. 
  • Ensures EFMP families are referred to the Military Family Readiness Center for additional community assistance as needed. Maintains a cooperative working relationship with the base Military Personnel Section (MPS), Commander Support Staff (CSS), and with Air Force Personnel Center (AFPC). 
  • Actively supports the integration of EFMP-M 
  • Exceptional Family Member Program – Family Support (EFMP-FS) and Exceptional Family Member Program – Assignments (EFMP-A) services at the installation. 
  • Provides information, appropriate contact information, and coordinates referrals as appropriate. 
  • Participates in EFMP quarterly case reviews to discuss assess newly identified families’, complex, or unmet clinical needs. 
  • Determine the appropriate resources necessary for the families. 
  • Patient specific assessments and clinical plans of action will be in AHLTA and the Special Needs record. Meeting minutes will not contain patient identifying information. 
  • Coordinates and participates in designated facility meetings. 
  • Coordinates and participates in designated facility meetings. 
  • Participates in the clinic orientation and training of other staff of the Special Needs Program. May serve on committees, work groups, and task forces at the facility. 
  • Must maintain a level of productivity and quality consistent with complexity of the assignment; facility policies and guidelines; established principles, ethics and standards of practice of professional nursing, Health Services Inspection (HSI); and other applicable DoD and service specific guidelines and policies.
  • Completes medical record documentation and coding, and designated tracking logs and data reporting as required by local MTF/AF/DoD instructions, policies, and guidance. 
  • Ensures compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the medical treatment facility. Provides care within ethical and legal boundaries. 
  • Serves and participates in committees, functions and other meetings as directed. Provides relevant and timely information to these groups and assists with decision making and process improvement. Participates in customer service initiatives, and medical readiness activities, designed to enhance health services. 
  • Completes orientation and competency verification programs in accordance with (IAW) unit guidelines. 
  • Participates in the orientation and training of newly assigned personnel, as appropriate. 
  • Obtains/maintains/documents professional Continuing Nursing Education (CNE) to comply with licensure and AF requirements, MTF initial and annual training requirements, and required unit-specific in-services/training. 
  • Implements evidence-based findings into daily practice, processes, and policies. 
  • Participates in/attends, but not limited to, mandatory organizational ancillary training programs; continuing educational programs to update and/or maintain the knowledge and skills needed to meet or enhance competency requirements; interdepartmental and unit-level in-services/briefings; continuing quality (i.e., 
  • Trusted Care, Team-STEPPS (Team Strategies and Tools to Enhance Performance and Patient Safety), 
  • Participates in peer review activities. 
  • Encourages team building and enhances work knowledge by cross training/augmenting other units as appropriate. 
  • Ensures a safe work environment, employee safe work habits and patient safety IAW regulatory agencies, infection control policies, and process improvement initiatives. 
  • Obtains/maintains/documents professional Continuing Nursing Education (CNE) to comply with licensure and AF requirements, MTF initial and annual training requirements, and required unit-specific in-services/training. 

 

Minimum Qualifications: 

  • Minimum, Bachelor of Science in Nursing.
  • Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE) or the American Nurses Credentialing Center (ANCC).
  • Advance Life Support.
  • Current Basic Life Support (BLS) for Healthcare Providers.
  • Three years of experience in nursing after graduation. 
  • Current, valid, active, and unrestricted license (with no limitations, stipulations, or pending adverse actions) in a state or territory of the United States or the District of Columba to practice as a Registered Nurse.
  • Knowledge of computer operations and proficiency in the use of basic word processing data entry and automated medical records. Strongly preferred working knowledge of the military health care system including electronic health records and computer systems. 
  • Must be a US Citizen.
