What are the responsibilities and job description for the COVID-19 COMMUNITY HEALTH WORKER position at PRAC of Southern New Jersey?

This is a full-time, temporary position. The Community Health Worker (CHW) will work in Salem County with a focus on Penns Grove, Pennsville and Salem City neighborhoods. The CHW will conduct community outreach and health education around COVID-19 and act as a bridge to COVID-19 testing, vaccine, and support services. This position is based at Penns Grove, NJ and will include some night and weekend work.

ESSENTIAL FUNCTIONS

· Outreach and health education around COVID-19

· Act as a bridge to COVID-19 testing, vaccine, and support services

· Perform other duties as required.

JOB REQUIREMENTS & QUALIFICATIONS

Education: Bachelor’s degree in social work or master’s degree preferred, but not required.

CHW Responsibilities:

· Conduct door to door COVID-19 outreach and engage residents in Salem County neighborhoods

· Provide education on COVID-19 prevent, testing, vaccination, and support services

· Maintain Log of activities and complete regular reports in order to meet grant objectives

· Works closely with the local Department of Health and other community-based organizations, providing strategy input and feedback

· Attend community testing and vaccination events to provide outreach and support

· Refer community members to services, resources, and information based on their stated needs

· Build and maintain relationships with community members and organizational partners

· Understands, implements and monitors program requirements.

· Meets and greets all visitors and potential new clients (individuals and families).

· Determines clients' requirements by completing intake interviews.

· Conducts needs assessments to present options and determine/recommend the best course of action.

· Cary out home visitation as necessary.

· Provides transportation services to testing and vaccination sites as necessary (driver)

· Maintains clients' files by reviewing case notes and recording progress

· Makes recommendations to PRAC’s leadership for program improvement

· Attends organizational (internal) and funding source’s meetings as scheduled

· Tracks and reports client services and contacts via program internal database (Master List), weekly, monthly and quarterly statistical and narrative reports

· Attends educational and training opportunities as required

· Knowledge and respect for diverse cultures/lifestyles and problems of poverty in the community.

· Ability to maintain the confidentiality of information on each client

· Skilled in use of computer applications such as word processing, custom database, web-based applications/processes and electronic filing

· Ability to communicate effectively with and relate positively to clients, agency staff, and other social service agencies

· Ability to be a self-starter and work independently of direct supervision

· Ability to follow written and verbal instructions

· Knowledge of general office procedures

· Ability to implement concepts learned from training

· Ability to flex schedule to meet service demands

· Ability to manage stress, organize time and work priorities effectively.

· Ability to do paperwork accurately, completely and timely

· Ability to communicate effectively both orally and in writing in English and Spanish

Experience

  • Must have experience working with low-income, culturally and linguistically diverse populations, in a fast-paced, deadline-oriented work environment.

  • This position is required to work weekends or other irregular hours as deemed necessary.   • Must possess a valid New Jersey Driver’s License
  • Must pass a pre-employment background investigation & pre-employment drug screen
  • Ability to work 40 hours per week
  • Ability to work nights & weekends if necessary
  • Fluent in the Spanish Language is preferred.

Job Types: Full-time, Temporary

Pay: $18.50 per hour
