Responsibilities
Responsible to provide case management to patients receiving care at PrairieCare, a Psychiatric HealthCare Organization. The Case Manager focuses on admissions, care coordination, communication, and aftercare planning.

Case Managers are supported by team leads, interdisciplinary team members, supervisors and directors.
- Provides case management services to patients receiving psychiatric services; supporting patients through admission to discharge
- Coordinates effective communication with patients, families, outpatient providers, and other involved agencies to promote the care of the patient
- Supports psychosocial perspective of patient's needs to the interdisciplinary team and collaboarates with all team members, adjusting plans as needed
- Connects patients with community resources to increase their long term overall well-being
- Provides education and support to patients in group settings
- Ensures compliance of documentation within progress notes, treatment plans, and safety plans
- Be oriented to ethically provide and advocate for patient-centered, culturally appropriate, and age appropriate care.
Qualifications
- Bachelor's in Psychology, Sociology, Human Services, or a closely related field required.
- Experience in healthcare setting preferred
- Experience using electronic health records preferred
- Knowledge of Microsoft Office Products including Word and Outlook
- Able to effectively and efficiently communicate in written and verbal form
- Ability to smoothly and rapidly shift between dissimilar tasks
- Following de-escalation training, ability to respond appropriately to patients experiencing a crisis
